Achievement Of The Maternal Role In Pregnancy During The Covid Pandemic

Abstract : Achieving the role of being a mother is a very important thing in a pregnancy and it requires an adaptation process. The COVID-19 pandemic situation which requires a change in the new habit order causes more stress for pregnant women in adapting to the achievement of their new role. This study aims to determine the achievement of the role of motherhood during the pregnancy period during the Covid 19 pandemic. This is a type of quantitative research with a cross sectional design. The number of research subjects is 99 taken from the entire population or total sampling. Data were collected using the instrument "What Being the Parent of a New Baby is Like: Revision of an Instrument (WPL-R)" and analyzed by logistic regression. Parity and occupation affect the achievement of the mother's role during pregnancy with a significance value of <0.05. Parity is the most influential factor on the achievement of the role of pregnant women in Bandarharjo Village, North Semarang, Semarang, Central Java with an OR value of 10.759 and p value of 0.000.

The Prevalence of Underweight, Stunted, And Wasted, and the Risk Factors That Influence It in Infants Under Six Months

Abstract : The prevalence of malnutrition is still a significant worldwide problem. WHO states that 45% of child deaths worldwide are related to undernutrition. The study aims to know the prevalence of underweight, stunted, and wasted and the risk factors that influence it in infants under six months. The study method is a cross-sectional analysis of infant nutritional status parameters in healthy infants under six months. The study was established at 10 Community Health Centers in Banjarbaru, South Kalimantan, Indonesia, from June 2020 to June 2021. This study recruited 423 healthy infants, 223 (52.7%) boys and 200 (47.3%) girls aged 1-6 months. The prevalence of underweight, stunted, and wasted were 0.9%, 0.5%, and 13.5%, respectively. There is a significant relationship between maternal age and infant nutritional status based on BMI for age (p=0.025). There is a significant relationship between maternal education and infant nutritional status based on weight for length (p<0.05). There is a significant relationship between maternal occupation and infant nutritional status based on weight for length and body mass index (BMI) for age (p<0.05). There is a significant relationship between family income and infant nutritional status based on weight for age and weight for length (p<0.05). There is no association between infant nutritional status, maternal parity, and exclusive breastfeeding practices. Conclusions: There is a significant relationship between maternal education, maternal occupation, and family income with undernutrition in infants under six months.

Association Between Level of Vitamin D with Degree of Disease Activity in Rheumatoid Arthritis

Abstract : Rheumatoid arthritis (RA) is a chronic inflammation, multisystemic autoimmune disease that accounts for nearly 18 percent of all deaths. Etiology is remain unknown, but interaction between genetic factor and environment can activate the immune response. DAS 28 is a scoring system to assess the disease activity of RA. Vitamin D is produce from UV exposure and dietary intake. Vitamin D can suppres the immune system by maintain the balance between TH-1 and TH-2 and also can reduce the inflammation by inhibit cytokine proinflammation. This research is a cross-sectional study, conducted from July 2022 to December 2022 at the Makassar rheumatology clinic, Dr. Wahidin Sudirohusodo and its network. The subject is patients with rheumatoid arthritis according to ACR/EULAR 2010 who meet inclusion criteria. The degree of disease activity was assessed by DAS 28, then divided into 3 groups (mild, moderate, and severe). 25-hydroxyvitamin D3 was measure by the ELISA (enzyme-linked immunosorbent assay) with results in ng/mL units of measure. Data analysis used SPSS version 22 and spearman correlation test. In this study, a value (rs: -0.998) was obtained, there was a strong relationship between vitamin D levels and degree of RA disease activity, which is the lower the vitamin D levels, followed by an increase in RA disease activity. An inverse relationship was found between vitamin D levels and the degree of disease activity in rheumatoid arthritis.

Liver diseases in pregnacy

Abstract : The focus of this article is on the most important (prognostic and therapeutic) pregnancy related liver diseases: cholestasis of pregnancy, preeclampsia induced liver disfunction and intrahepatic cholestasis of pregnancy, eclampsia (including HELLP – hemolisys, elevated liver enzymes, low platelet count) and acute hepatic steatosis. They are completely reversible after delivery and must be distinguished from other intercurrent liver diseases, for which pregnancy is a possible precipitating factor. Intrahepatic (obstetric) cholestasis of pregnancy (ICP) is a unique hepatobiliary condition that manifests itself with itching, jaundice, an increase in the serum concentration of bile acids and poses a risk of complications for both the mother and the baby. ICP is a reversible type of hormonally influenced cholestasis. It develops during the third trimester, when hormone concentrations are at their peak. Ursodeoxycholic acid relieves pruritus and improves liver function tests in pregnant women in intrahepatic cholestasis and largely reduces fetal mortality, but has a secondary role in HELLP syndrome and acute hepatic steatosis during pregnancy, where timely delivery is the key to treatment.

The Influence of Spirituality in The Workplace and Spiritual Leadership On Performance at Employees of PT Bumi Jasa Utama Makassar

Abstract : Human resources are one of the most vital elements in an organization. Similar to companies as organizations require employees who can work better and faster, thus employees who have maximum performance are required. Many factors influence the employee’s performance such as decreased commitment and job satisfaction. These problems can be mitigated in several ways such as by applying and encouraging spirituality in the workplace in particular by a leader. Based on this, researchers are interested in assessing whether there is an influence of workplace spirituality and spiritual leadership on the performance and job satisfaction of PT Bumi Jasa Utama Makassar employees. This research used a cross-sectional study design and was conducted in November 2022 with a sample of 50 employees. Data analysis used Path Analysis to see the direct effect as well as the influence moderated by the moderating variable (job satisfaction). Path analysis results show that spirituality in the workplace (p=0.040) and spiritual leadership (p=0.030) have a relationship or influence on employee performance. Path analysis results also conclude that job satisfaction as a moderating variable is capable of moderating the influence of workplace spirituality and spiritual leadership on employee performance (p<0.05). Activities are needed that improve the quality of relations between employees and leaders to create a good spiritual state to maximize employee performance.

Complex Management of Acute Variceal Bleeding

Abstract : Acute variceal bleeding remains with a high mortality rate (around 15 %). Treatment is based on the combined use of vasoactive drugs, endoscopic band ligation and antibiotic therapy. Effective resuscitation (blood transfusions, volume replacement) is essential to prevent complications. In case of failure - transjugular intrahepatic portosystemic shunt (TIPS) with appropriate indications and limitations related to the prognosis of the individual patient. Balloon tamponade or specially designed coated esophageal stents can be used as a bridge to definitive therapy in unstable patients. Early TIPS should be the first choice in patients at high risk of treatment failure (Child-Pugh B with active bleeding or Child-Pugh C < 14). This article discusses the latest advances in variceal bleeding management in line with current recommendations of the Baveno VI Consensus Conference.

The Risks of Patient Care Against Covid-19 Incidents in Health Workers at the Manembo-Nembo Hospital in Bitung, North Sulawesi Province

Abstract : The Coronavirus disease 2019 (Covid-19) pandemic caused by severe acute respiratory syndrome SARS-CoV-2 has challenged the health systems of countries affected by it. Covid-19 poses an important occupational health risk for health workers. Health workers are the front line in handling the Covid-19 outbreak putting them at risk of infection. This study aims to determine and analyze the risk of treatment for the incidence of Covid-19 in health workers. This research method used a Case Control Study design and was carried out at the Manembo-Nembo Hospital in Bitung, North Sulawesi Province. A sample of 96 people consisting of 48 cases and 48 controls. Sampling was purposive sampling, data collection techniques using questionnaires were applied using kobotoolbox software. Data analysis using the STATA 14 program. The results of the study a significant risk factor for the incidence of Covid-19 was a history of direct patient contact (OR=14.33 95%CI:4.4-53.2; p=0.000), face-to-face (OR=11,66 95%CI: 3.8-39.2; p=0.000), Direct contact with the environment where the patient is treated (OR= 4.42 95% CI: 1.73-11.4; p=0.000). Vaccination status (OR=2.04 95% CI: 0.10-123.06; p=0.557), Comorbid (OR=0.37 95% CI: 0.34-2.4; p=0.238), time of work (OR=0.89, 95% CI: 0.31- 1.53; p=0.811) not a risk factor. The most dominant factor at risk for the occurrence of Covid-19 is a history of direct contact with patients who have been confirmed to have had Covid-19. The high prevalence of health workers who have confirmed Covid-19 has a history of contact with patients with confirmed Covid-19.

Effect of Vitamin D3 2000 IU Tablets on Serum Vitamin D 25-OH Levels, Hemoglobin Levels, Body Mass Index and CD4/CD8 Ratio in Patients Infected with Human Immunodeficiency Virus

Abstract : To assess the effect of administration of vitamin D3 tablet therapy on serum vitamin D 25-OH levels, hemoglobin levels, body mass index (BMI), and CD4/CD8 ratio in HIV/AIDS infected patients who have been treated with antiretrovirals (ARV). This study was a cross-sectional study using 40 samples of HIV patients who were routinely controlled at the VCT polyclinic at Dr. Moewardi who received regular antiretrovirals. The results of testing the mean difference between the two groups of serum levels of vitamin D 25-OH in the Vitamin D3 treatment and control groups showed a value of Z = -2.277 (p = 0.023). The results of the different test mean that the two groups of hemoglobin in the Vitamin D3 treatment group and the control showed a value of Z = -3.484 (p = 0.001). The results of different tests mean that the two BMI groups in the Vitamin D3 treatment group and the control group show a value of t = -5.098 (p = 0.001). The results of the mean difference test of the two CD4/CD8 ratio groups in the Vitamin D3 treatment group and the control group showed a value of t = -6.203 (p = 0.001). Administration of 2000 IU vitamin D3 tablets can increase serum vitamin D 25-OH levels, hemoglobin, BMI, and CD4/CD8 ratio in HIV/AIDS infected patients who have been treated with antiretroviral (ARV).

THE EFFECT OF WORKLOAD AND WORK STRESS WITH FATIGUE ON THE WORK PERFORMANCE OF NURSES AT DADI SPECIAL HOSPITAL, SOUTH SULAWESI PROVINCE

Abstract : Workload and work stress with fatigue are some of the risk factors for work performance problems. This study aims to determine the effect of workload, work stress’s and fatigue and their impact on the performance of nurses at Dadi Hospital, South Sulawesi Province. The research was conducted at the Dadi Hospital in South Sulawesi Province. This research uses quantitative methods. The population in this stuwasere 233 nurses at the Dadi General Hospital in South Sulawesi Province with 147 nurses used as samples. The type of research in this study is Analytical Observational using Cross-Sectionalal approach and this research uses a Path Analysis model, in which the intervening variable is fatigue which lies between exogenous variables. The data that has been collected is then analyzed through a process of data reduction, data presentation, and drawing conclusions and verification. From the results of the study it can be concluded that 46 respondents (31.3%) experienced heavy workload, 82 respondents (55.8%) experienced moderate work stress, 105 respondents (71.4%) were very tired), and poor work performance of 59 respondents (40.1%). Based on the results of statistical tests using the chi-square test, thp-valueue was 0.007 <0.05, meaning that there is a significant relationship between work stress and fatigue. And based on the results of AMOS work stress affects performance through fatigue (0.011). From the results of the study it can be concluded that 46 respondents (31.3%) experienced heavy workload, 82 respondents (55.8%) experienced moderate work stress, 105 respondents (71.4%) were very tired), and poor work performance of 59 respondents (40.1%). Evaluation of Trunk Muscle Activities in Response to Four Common Infant Carrying Methods Utilized by African Women

Abstract : Infant carrying has been reported to be physically demanding with risks of physical challenges to mothers/caregivers. This makes it important to evaluate different Infant Carrying Methods (ICMs) to guide choices for the promotion of maternal health. This study evaluated the effects of four ICMs (A–back, B–front, C–side, and D-in-arms) on trunk muscular activities. Thirty women simulated four ICMs while the electrical activities of the right (Rt) and left (Lt) components of the erector spinae (ES), rectus abdominis (RA), and external oblique (EO) muscles were recorded simultaneously during the tasks. Within-subject comparisons of the muscle activities showed that only the front (p = 0.011) and side (p = 0.033) ICMs elicited significant differences with their highest activities recorded in the left and right EO muscles, respectively. Between-subject comparisons of the muscle activities showed a significant difference (p = 0.022) only in the right EO muscle, being most active during the side ICM. The side ICM was observed to elicit the highest activities in three of the four studied muscles. Side ICM has more potential for over-exerting the trunk muscles, which could be a possible factor for muscular fatigue and resultant musculoskeletal impairments.
