Implementation of Convergence Action on Stunting Program in North Toraja Regency

Abstract : Convergence action on stunting program is well organized, integrated and together, by involving cross-sectors to prevent and accelerate the reduction of stunting. This study aims to analyze the implementation of convergence action on stunting program in North Toraja Regency. The design of this study used qualitative research method with a descriptive-exploratory approach through observation, in-depth interview and literature study of all informants which selected by purposive sampling technique. The variable of this study used the theory of policy implementation by Edward III (1980) which includes communication, resource, attitude/disposition and bureaucratic structure. The results of the study show that the implementation of convergence action on stunting program in North Toraja Regency based on the communication aspect, it still needs to be maximized to the community. Likewise, with the aspect of resource, the availability of human resource in the health and agriculture sectors has not been fulfilled, limited budget for Intervention Implementation, and standardized service facilities in the health sector have also not been fulfilled. The commitment and support of the regional leadership have been gone well, but it must still be supported by the attention and seriousness of OPD in carrying out their own duties and responsibilities.

Analysis of Behavioral Determinants of Pregnant Women on Tetanus Toxoid (TT) Immunization in Indonesia

Abstract : Tetanus neonatorum is one of the crucial health problems, especially in countries with low coverage of Tetanus Toxoid (TT) immunization such as Indonesia. In 2020, the coverage of TT immunization for pregnant women in Indonesia decreased from 2019. This study aims to determine the behavioral determinants of pregnant women toward TT immunization. This research is a quantitative study with an analytic design with a cross-sectional approach. The questionnaire was filled out by 137 pregnant women in the District of West Tapalang. The data from this study were analyzed using the chi-square test and multiple logistic regression. The results indicated that there were two determinants related to the behavior of pregnant women towards TT immunization, including knowledge (p=0.018) and perception of barriers (p<0.001). Multivariate analysis showed that the dominant determinant influencing the behavior of pregnant women towards TT immunization was the perception of barriers (OR=10.046). Therefore, only the knowledge and perceived barrier variables were related to the behavior of pregnant women towards TT immunization, where the perceived barrier was identified as the dominant variable. It is hoped that the Puskesmas staff will increase the coverage of TT immunization for pregnant women by routinely sweeping the status of TT immunization and providing education about TT immunization by involving husbands as an essential target in the education process.

The Effectiveness Of Moringa Seed Coagulant Combination (Moringa Oleifera), Pumice Stone Filter And Activated Mangrove Charcoal (Rhizophora Sp.) In Reducing Tds Level In Barrang Caddi Island, Makassar City

Abstract : The limited clean water in small island is caused by contamination of sea water to clean water sources of society. This study aims to investigate the effectiveness of several methods of brackish water processing to reduce TDS level. This study is one group pretest-posttest design experiment, this study used water processing with coagulation method of Moringa seeds at dose of 350 mg/l, a pumice stone filter with thickness of 60 cm, an activated carbon filter (mangrove wood) with thickness of 60 cm, and combination method of coagulation-filtration in reducing Total Dissolved Solid (TDS) level. The sample of this study was dug well in Barrang Caddi Island, the data were analyzed using the effectiveness formula, Kruskal Wallis test. Brackish water processing with the coagulation method of Moringa seeds showed an average value of the effectiveness in reducing TDS level of 41.38%; pumice stone filtration of 28.82%; activated charcoal filtration was 63.53% and the combination of coagulation-filtration was 54.66% and there was a significant difference in the effectiveness between the four water treatment methods on TDS parameter (p = 0.016).There is fairly good effectiveness on the combination of coagulation and filtration method in reducing TDS level.

The Effectiveness of Administration of Multiple Micronutrient Supplement (MMS) Tablets on Hemoglobin Levels of Pregnant Women in Pare-Pare City, Indonesia

Abstract : The World Health Organization (WHO) states that anemia is the 10 biggest health problem in this modern century, where groups at high risk of anemia are women of childbearing age, pregnant women, school-age children, and adolescents. This study aims to analyze the effectiveness of Multiple Micronutrient supplements (MMS) tablet administration on hemoglobin levels of pregnant women. This study used a quantitative research type with a quasi-experimental approach using a group pre-post test study design, mean/median analysis, and paired sample t-test analysis. The research was conducted for three months at the Lapadde Health Center and Lumpue Health Center in Pare-Pare City with a sample of 310 respondents. The inclusion criteria in this study were third-trimester pregnant women consuming MMS from the beginning of pregnancy, having initial Hb data, and having consumed MMS for at least 1 month. Check Hb using the Easy Touch tool. The incidence of anemia was measured based on hemoglobin levels using the Easy Touch tool, and data on characteristics and MMS supplementation were collected through a questionnaire that was filled in directly by the respondent and through the interview method. The results of the Chi-Square test showed that the consumption of MMS tablets was significantly associated with the incidence of anemia in pregnant women. (p<0.05) Pregnant women who experienced anemia did not regularly consume MMS every day as much as 26% and 79.6% liked to consume MMS tablets. There was a change in hemoglobin levels of pregnant women before consuming MMS and after consuming MMS, where The hemoglobin of pregnant women before consuming MMS was 12 g/dL ± 1.583 SD and after consuming MMS was 13.64 g/dL ± 2.047 SD with P ≤ 0.05. Further research is needed by adding other variables, in this case looking at consumption patterns of pregnant women and other factors related to hemoglobin levels of pregnant women and can be considered for conducting pure experiments.

Association between Estradiol Levels and Central Corneal Thickness in Primary Open Angle Glaucoma (POAG) Patients

Abstract : Primary open angle glaucoma (POAG) is the most common form of glaucoma. The risk of developing glaucoma in women is influenced by several factors, including sex hormones. Estrogen is proven to protect against the damage from glaucoma. Estrogen monotherapy is suggested to associate with a small but significant reduction in intraocular pressure (IOP) in postmenopausal women. To determine the relationship between estradiol levels and central cornea thickness (CCT) in POAG. A case control study conducted at Ophthalmic Clinic in Universitas Sumatera Utara Hospital and Network Hospital, from February 2022 until fulfilment of sample number. The samples were women with POAG, who met criteria inclusion and exclusion. Data were collected and analyzed. From 30 subjects, the mean age of POAG grup was 55.87 years, with the youngest and the oldest being 40- and 85-year-old respectively. There was no significant relationship among age category (p=0.573), as well as IOP, visual field defects, cup disc ratio and CCT, with estradiol levels in both POAG and control groups (p>0.05). Central thickness was not related to estradiol levels in POAG patients.

Association Between Plasma Selenium Levels With The Event Of Primary Open-Angle Glaucoma

Abstract : Glaucoma is the leading cause of irreversible blindness and the second most common cause of blindness after cataracts. Primary open-angle glaucoma (POAG) is the most common form of glaucoma. Selenium is thought to be a factor that influence POAG’s development. This study aims to determine the association between plasma selenium levels and POAG. Analytical observational study was used in this study. A total of 60 subjects were divided into case and control groups with the same amount of subjects. Case group was subject diagnosed with POAG and control group was subject with normal eyes. The samples were consecutively recruited from February 2022 until the number of samples fulfilled in Glaucoma Clinic at Opthalmology Department, University of Sumatera Utara Hospital. Subjects in both groups underwent eye examinations and 3 ml of venous blood samples was taken to analyze selenium levels in the subject’s plasma, then recorded and statistical analysis was performed. Most subjects in POAG group were male, with 16 subjects (53.3%) in the age of 40 to 85 years and the mean age was 55,87 years old. The mean Selenium levels in the POAG group was 328.64 ng/dL, with the lowest value being 104.58 ng/dL, and the highest value 1017.8 ng/dL. There was a significant difference in plasma Selenium levels in POAG and control group, and there was a relationship between Selenium and IOP in the POAG group, but no correlation was found between Selenium and other parameters.

Perceptions and Attitudes of Pharmacists toward Online Pharmaceutical Services

Abstract : Advancements in technology and information have driven the development of online pharmacy services, but this matter still generates controversy, as perceived by pharmacists. The research analyzes the relationship between pharmacist perceptions and attitudes toward online pharmaceutical services. This research is analytical observational research with a cross-sectional design. This research sample comprised 105 pharmacists with prior knowledge of online pharmaceutical services and practices in pharmacies in the Greater Malang area (Indonesia). The utilized research instrument was questionnaires on pharmacist perceptions and attitudes toward online pharmaceutical services. The used correlation test was the Spearman test. The research results indicated that pharmacists possess positive (44%) and negative (56%) perceptions, as well as positive (37%) and negative (63%) attitudes toward online pharmaceutical services. From the correlation testing, it was found that there was a significant relationship between the perceptions and attitudes of the pharmacist toward online pharmaceutical services (p = 0.000). The research conclusion is that pharmacists in the Greater Malang area possess negative perceptions and unsupportive attitudes toward online pharmaceutical services, by which perceptions and attitudes are related significantly and directly.

Relationship between Glomerular Filtration Rate and α-Klotho Levels in Chronic Kidney Disease: Sub-Analysis CKD-MBD Study

Abstract : α-Klotho is a co-receptor of fibroblast growth factor-23 (FGF-23) to carry out its biological function. αKlotho is produced mainly in the kidneys so that with decreased kidney function (decreased kidney mass), the production of α-Klotho decreases. Several other factors affect α-Klotho levels including age, BMI, DM, and hypertension. This study aims to determine the relationship between α-Klotho levels at various stages of Glomerulus Filtration Rate (GFR) in CKD. Observational research with a cross sectional approach at Wahidin Sudirohusodo Hospital and Unhas Hospital Makassar since April - Agustus 2021. Subjects is CKD patients which meet inclusion criteria. Intact serum FGF-23 levels were measured using an ELISA kit (Immutopics). Statistical analysis using Annova test, Mann Whitney and Spearman's correlation test, Statistical results are considered significant if p value < 0.05. This study included 77 subjects of CKD stage 3, 4, and 5 with a distribution of 39 men and 38 women. The results of the analysis found a decrease in-Klotho at each increase in CKD stage (p<0.05). The correlation shows that the lower the GFR levels, the lower the levels-Klotho (p<0.05). the level of α-Klotho is determined by the age factor (p<0.05). The decreasing of GFR is followed by decreasing of α-Klotho levels.

POLYMORPHISM I/D GEN ACE-1 INCREASE THE RISK OF SEVERE KNEE OSTEOARTHRITIS IN OLDER ADULT: A PRELIMINARY STUDY

Abstract : It has been shown that the interplay of degenerative and inflammatory processes has a role in the etiology of osteoarthritis. The Renin-Angiotensin System (RAS) has reported in knee OA pathophysiology and ACE level influenced by ACE I/D polymorphism. This study aimed to examine the influence of the interaction between age and the DD genotype of the ACE gene on knee OA severity. A total of 80 knee OA patients (mean age 62.9 ± 8.84 years) were included in this cross-sectional study. The severity of knee OA was determined based on the Kellgren–Lawrence system. The Kolmogorov–Smirnov test, chi-square test, and logistic regression multivariate analysis were employed in statistical analysis. This study included 80 female subjects: 40 subjects with mild knee OA and 40 subjects with severe knee OA. The mean age of 63.7 ± 7.7 years was found for 34 subjects (42.5%) aged ≥65 years and 46 subjects (57.5%) <65 years. Based on the ROC curve, the cut-off age is 65 years. The DD gene ACE-1 genotype were not proven to increase the risk of severe knee OA. The DD gene ACE-1 genotype increases the risk of severe OA of the knee by 4 times in subjects aged ≥65 years compared to subjects <65 years. Our study found that the DD genotype ACE-1 did not increase the risk of severe knee OA. At age 65 years, the DD genotype ACE-1 enhances the risk factors for knee OA severity.

Mental Disorders with Sida Cultural Background in the Manggarai Tribe East Nusa Tenggara

Abstract : The Manggarai community adheres to a patrilineal system (following the father's lineage). Sida is a relative of the wife's origin, called anak rona, while anak wina is the receiving family of the wife. The sida culture is closely related to the life of the Manggarai tribe. In the culture of sida contained values that are full of meaning. In this case series, we discuss two cases of neurotic disorders that have a Sida cultural background in the Manggarai tribe. It is very important to identify the influence of culture on mental disorders so that they can provide a more comprehensive treatment. The first patient thought about the anxiety that was felt at times. The second patient felt anxious thinking about the headache. He felt that there was something in his head like a brain tumor. It was found that people with mental disorders in the United States had a worse prognosis than those in India. This is because in the United States, people with mental disorders experience a heavier stigma. Culture have multidimensional effect on mental health. Culture can be a supporting factor or an inhibiting factor in determining the prognosis of mental disorders.
