Echocardiographic Evaluation of Patients with Rheumatoid Arthritis

Abstract : Rheumatoid arthritis (RA) is a chronic inflammatory disease of the joints that may also manifest with extra-articular characteristics. Cardiovascular disorders are among those extra-articular characteristics, including pericarditis, cardiomyopathy, myocarditis, cardiac amyloidosis, coronary vasculitis, arrhythmia, valve diseases, and, most significantly, congestive heart failure and ischemic heart disease. RA is linked to a higher death rate when compared to the general population, the bulk of which is caused by cardiovascular illnesses. The aim of this study is to assess the prevalence of Echocardiographic findings in patients with an established diagnosis of rheumatoid arthritis. A total of 50 patients (42 female: 8 male) diagnosed to have RA according to the ACR/EULAR classification criteria for Rheumatoid arthritis were involved in this cross-sectional study. Data collection was done using questionnaires and interview. Age, gender, disease duration, current medications, rheumatoid factor, anti CCP and Hemoglobin concentration. All subjects were evaluated by Trans-thoracic Echocardiography to determine cardiac functional and structural abnormalities. Mean age for patients was 49.53± 11.55 years. Echocardiographic abnormalities were found in 26 (52%). Most common echocardiographic abnormality was diastolic dysfunction. This study shows significant prevalence of cardiac involvement in RA patients.

Relationship between Troponin Level and Systolic and Diastolic Dysfunction in Children with Sepsis

Abstract : Sepsis induced myocardial dysfunction (SIMD) increases mortality risk to 70-90% compared to sepsis without cardiovascular system dysfunction. Troponin is a specific and sensitive marker for myocardial injuries, where high troponin levels are frequently reported in critically ill adult patients. However, very few studies are available on systolic and diastolic heart functions in child patients with sepsis in relation to troponin levels. This was a prospective cohort research to assess the troponin levels and the relationship with systolic and diastolic dysfunction at 24 and 72 hours of pediatric sepsis at the PICU in Haji Adam Malik and Universitas Sumatera Utara hospitals Medan, Indonesia. The relationship between the mentioned variables was assessed using paired t-test for normal distribution or Mann-Whitney test for non-normal distribution data. Research subjects were children aged between 1 month to 18 years old that fulfilled sepsis criteria according to PELOD-2 scoring system. Patients with congenital heart diseases and myocarditis, as well as post-heart surgery patients were not included in the study. The frequency distribution of systolic and diastolic function showed no systolic dysfunction in patients. However, diastolic function assessment showed 8 patients (26.7%) with diastolic dysfunction on the first day of sepsis diagnosis, and the number increased to 11 patients (36.7%) on the third day of sepsis diagnosis. Troponin I and T showed a significant relationship with diastolic dysfunction (p<0.05). Troponin T and troponin I values measured on the first 24 and 72 hours of sepsis diagnosis or PICU treatment can indicate diastolic dysfunction.

Sunrise Syndrome: A Case Report

Abstract : Sunrise syndrome is a complication that most often occurs after cataract surgery. This complication occurs in cataract surgery with improper fixation of the IOL inside the capsular bag, or incomplete capsulorhexis. Overall, the incidence rate of Sunrise syndrome has actually decreased a lot with the increasing use of curved capsulorhexis and phacoemulsification with the safe placement of IOL in capsular bag [1]. To report the case of a patient who had sunrise syndrome. Woman 52 years old with main complaint blurry vision in both eyes since 3 months ago. Visual acuity examination revealed the visual acuity of Hand movement on the right eye. There was no improvement with pinhole, and the vision of the left eye was 0.20 with a pinhole of 0.40. Intraocular pressure in the right eye was 17 mmHg. The anterior segment was within normal limits, except the lens appeared cloudy. The patient was diagnosed with mature senile cataract on her right eye and underwent cataract extraction with the phacoemulsification method. Postoperative visual acuity phacoemulsification H+7 the patient complained of glare sometimes like seeing a rainbow, and occasional double vision in the eye that had been operated on. On visual acuity examination, the vision was 0.17, and there was a shift in the IOL lens. In patients with sunrise syndrome, treatment with the capsular tension ring (CTR) approach is quite effective, with the result IOL in a central position and visual progress.

Effects of Training of Rice Farmers in OHS and Use of PPE on The Occurrence of Occupational Accidents and Rice Production

Abstract : Accidents among rice farmers are on the rise. We conducted a two year quasi experiment to investigate the effects of training on occupational health and safety standards (OHS) and use of personal protective equipment (PPE) on the occurrence of occupational accidents (OA) and rice production by rice farmers in rural Java Island, Indonesia. We randomly chose 70 rice farmers from adjacent villages within one sub-district of Sleman Regency. These farmers were randomly allotted to two groups of thirty-five people; both groups were given PPE since the first year, later on the second year, only group one were trained on OHS and use of PPE while the other group (C) were left untrained. We used descriptive analyses for use of PPE and an independent t-test was used to analyse variables, including cases of OA, age of farmers, education level and rice production. The results show that both OHS and the use of PPE enabled group T to have significantly (P<0.01) lower cases of OA than that of the control group C(1.57 vs. 2.38), leading to longer productive time to farm for the former group. Interventions on group T effectively (P<0.05) reduced risks of accidents or illness during rice farming by 75%. Group T produced much more (P<0.001) rice than group C (7,32 vs. 3,71; tonnes/ha); possibly because group T had a longer productive time. OHS training and use of PPE significantly reduced risks of OA and increased rice production.

KNOWLEDGE ABOUT CERVICAL CANCER AMONG URBAN AND RURAL WOMEN IN THE EAST COAST OF MALAYSIA

Abstract : Cervical cancer is the third leading cause of death among at-risk reproductive women in Malaysia. This study was to find out the level of knowledge about cervical cancer among urban and rural women. The study involved women aged between 30 and 65 years. The design of this comparative study was conducted to compare the level of cervical cancer knowledge among urban and rural women. The sampling method uses a probability sampling method based on sampling criteria and sample exclusion criteria. The IBM Corp.'s 2013 Statistical Package for Social Sciences (SPSS) version 20 software was used to analyse the data in this study. The results show that the percentage of this knowledge is good for urban and rural women on the east coast of Malaysia.

Clinical Approach in Generalized Pustular Psoriasis : One Case Report

Abstract : Generalized pustular psoriasis (PPG) is a rare subtype variation of pustular psoriasis. No specific therapeutic agent for PPG has been approved in the United States and Europe yet. Standard guidelines for PPG therapy do not yet exist, so therapy is determined based on the clinical. A case is reported, a 22-year-old woman with complaints of purulent papules on almost the entire body for 1 week ago. The patient had a history of red scaly patches for 3 years ago. On generalized regional dermatological status, diffuse miliary pustules were found on an erythematous basis with desquamation. Histopathological results showed subcorneal pustules with segmented (spongiform) neutrophil inflammatory cells infiltration. The patient was diagnosed as PPG and treated with methotrexate 7.5 mg/week. The patient showed significant lesion improvement within 14 days of treatment.

Knowledge of Safe Handling of Chemotherapy among the nurses in Queen Elizabeth Hospital, Kota Kinabalu, Sabah

Abstract : There are various types of cytotoxic agents used in the treatment of chemotherapy, and each agent can cause undesirable side effects in both patients and nurses, who are also skilled in dealing with these antineoplastic agents. Twenty true or false, closed-ended questionnaires were utilised to assess and evaluate the knowledge of nurses regarding the safety of chemotherapy. A total of 107 nurses participated in the assessment and evaluation, with only 59.22% receiving correct answers overall. The majority of respondents, 73.8% (79 out of 107) demonstrated moderate knowledge with 10 to 16 correct responses out of 20 questions. The majority of their knowledge regarding the safe handling of chemotherapy came from Continuous Nursing/Medical Education (CNE/CME) and the internet respectively. In general, the results demonstrated that nurses have insufficient knowledge of cytotoxic agents, as evidenced by the fact that the average percentage of correct responses was below 60%. Therefore, nurses must receive extensive additional education about anticancer agents in nursing school and through in-hospital continuing education. It is essential for nurses to participate in a structured educational programme, such as a Post Basic or Advanced Diploma in Oncology Care, in order to obtain proper updates and certification. In addition, the study suggested that updating the knowledge of safe handling among nurses with supported guidelines and policies will aid in enforcing safety implementation within clinical settings.

Changes In Clean Living Behavior In Mentally Retarded Children: An Quasi-Experimental Study Using The Flashcard Method

Abstract : Children with mental retardation have a high vulnerability to exposure to diseases such as Covid-19. Special interventions are needed to increase the knowledge of mentally retarded children so that they can practice handwashing skills with soap so as to minimize the transmission of Covid-19 and other diseases. One method that can be applied in the intervention of handwashing with soap is the drill method with flashcard media. This study aims to examine the effect of the flashcard-based drill method intervention on increasing the ability of mentally retarded children to wash their hands with soap. This study uses a Quasi-Experimental design. The planned design is a one group before and after intervention design. Based on the results of the bivariate test, it is known that there is a significant effect of flashcard-based drill method education on improving hand washing skills with soap in mentally retarded children, Samatiga District, West Aceh Regency. Research clearly shows that the flashcard-mediated drill method intervention has an effect on increasing the knowledge of mentally retarded children so that they are able to practice hand washing behavior with soap so as to minimize the risk of contracting Covid-19 and other bacteria.

Blood Transfusion Effect on Plasma Potassium Level in Pediatric Patients with Beta Thalassemia Major

Abstract : Thalassemia is the inability of the bone marrow to synthesize hemoglobin. Blood transfusion and chelation therapy is the treatment for beta thalassemia major. However, hyperkalemia is a common side effect because stored blood has elevated potassium level. In light of the significance of blood transfusion therapy in beta thalassemia major patients and the risk of hyperkalemia, this study was carried out in to investigate the effect of transfusion therapy on the changes in potassium levels, particularly in beta thalassemia major pediatric patients. This research was an observational paired numerical analysis conducted to evaluate the effect of blood transfusions on plasma potassium levels in beta thalassemia major pediatric patients at Hematology Oncology Haji Adam Malik Hospital. Blood sampling to test the plasma potassium levels was carried out before and one hour after transfusion. Spearman correlation test was used to determine whether blood storage and blood transfusion volume affected potassium level in the plasma. Paired T-test and Wilcoxon tests were used for the correlation analysis. Most research subjects (32/44, 72.7%) had increased plasma potassium level after transfusions. The mean potassium levels increased from 3.80 mEq/l to a mean value of 5.8 mEq/l after blood transfusions. There was a difference in potassium level between before and after transfusion (p<0.001). The volume of transfused blood did not significantly affect potassium level (p = 0.597, r = -0.082). Similarly, the storage time also did not show significant relationship with potassium level (p = 0.509, r = -0.102). The increase in potassium level in beta thalassemia major was not correlated with the volume and storage time of blood transfused.

The Effect of Long Storage of Thrombocyte Concentrate Blood Components on Beta Thrombomodulin Levels, Thrombocyte Number and Mean Platelet Volume (MPV)

Abstract : The process of platelet storage lesion during the storage of thrombocyte concentrate (TC) blood components can have an impact on platelet viability and decreased hemostatic function. This study analyzed the effect of storage time for TC blood components on beta thrombomodulin levels, platelet count, and mean platelet volume (MPV). This type of research is an analytical study with a time-series design. Sampling was done by consecutive sampling. A sample of 25 TC bags was freshly made and stored at room temperature with agitation in the Blood Bank of Dr Soetomo Hospital. Examination of beta thrombomodulin levels, platelet count, and MPV was performed to assess platelet storage lesion on day 1, day 3, and day 5. Data were analyzed using the Shapiro-Wilk test, Kruskal Wallis test, Bonferroni test, and Games Howell. There is a change in beta thromboglobulin levels, on day 1 the mean rank (32.5), day the 3rd (32,20), day 5th (49.24) and MPV, on day number 1 average (7,744 fL), day 3 (7,928 fL), day 5 (8,248 fL). There was no significant change in the platelet count on day 1 (mean (210,200 / uL), day 3 (192,485) /uL), 5th day (214.201 /uL). There was no decrease in the number of platelets on storage days 1, 3, and 5 so the TC component was still feasible to be given to patients, even though there were changes in beta thrombomodulin and MPV. Further research is needed for other parameters that can be affected by the storage process.
