Journal ID : AMJ-26-07-2022-10311; Author : Oleksii Korzh,
Abstract : The development of chronic kidney disease (CKD) is one of the most common conditions comorbid with chronic obstructive pulmonary disease (COPD). The aim of the study was assessment of renal blood flow in patients with COPD with and without CKD. 142 patients with COPD of 2-4 degrees were examined. The assessment of renal blood flow was carried out using the ultrasound by color Doppler mapping. All examined patients were divided into 2 study groups: Group I consisted of 71 patients with COPD with eGFR > 60 ml / min / 1.73m2, group II consisted of 71 patients with COPD with eGFR ≤60ml/min/1.73m2. The progression of kidney dysfunction in patients with COPD was accompanied by a significant decrease in exercise tolerance, worsening of the clinical condition of patients according to the clinical assessment scale and COPD assessment test with a decrease in the functional activity of patients. In COPD patients with CKD, changes were also noted at the level of the lobar and segmental renal arteries, characterized by a significant increase in pulsatile and resistance indices as well as there was a decrease in speed indicators during diastole, systole, and the average blood flow velocity. The COPD patients with renal dysfunction had changes noted at the level of the lobar and segmental renal arteries, characterized by a significant increase in pulsatility and resistance indices. Moreover, there was a decrease in speed indicators during diastole, systole, and the average blood flow velocity.

Isolation and Identification of Helicobacter pylori in Patient Suffering from Laryngeal Squamous Cell Carcinoma and Benign Laryngeal Polyps
Journal ID : AMJ-24-07-2022-10308; Author : Wasan Abdul-elah Bakir, Mais Ibrahim Alsikafi, Refif Sabih Al-Shawk,
Abstract : Helicobacter pylori has been detected in the laryngeal areas in clinical investigations. As a result, the goal of this study was to isolate and identify H. pylori infection in laryngeal biopsy from patients with benign laryngeal polyps (BLP) and laryngeal squamous cell cancer (LSCC) as compared with healthy people (control). A study was conducted on 50 patients suffering from squamous cells carcinoma of the larynx and 36 patients suffering from benign laryngeal polyps (BLP) compared with 12 healthy people or control. Highly significant (P<0.01) variations in H. pylori infection among LSCC, BLP and control that recorded 42%, 30.5% and 16.6%, respectively. mRNA IHC expression showed a highly significant elevation of LSCC patients and record 71.2 ± 4.2 as compared with 20.6 ± 1.1 recorded to BLP patients. IL-17 expression by IHC technique with and without H. pylori showed highly significant elevation (P<0.01) in the LSCC group compared with BLP and control. Correlation of IL-17 protein expression among the studied group with and without H. pylori showed significant (P<0.05) elevation in LSCC and BLP IL-17 level as compared with control. H. pylori can inhabit the larynx of the patients having laryngeal squamous cell carcinoma and benign laryngeal polyps, according to the findings. More investigation is required, however, to determine the role of H. pylori as a risk factor for disorders of the larynx.

Periosteum Preservation Approach In Endoscopic Sinus Surgery Before Subantral Bone Augmentation
Journal ID : AMJ-19-07-2022-10307; Author : Volodymyr Shkorbotun, Yaroslav Shkorbotun:,
Abstract : To evaluate the effect of periosteum preservation approach in endoscopic sinus surgery on subantral bone augmentation in patients with the maxillary sinus pathology. The results of subantral bone augmentation of 245 patients who underwent surgery for the paranasal sinus pathology (chronic rhinosinusitis, fungus ball and maxillary sinus cysts) were studied. In 146 patients endoscopic sinus surgery was performed with periosteum preservation approach (main group), and 99 patients underwent traditional surgery (control group). The total proportion of patients who had the perforation of the Schneiderian membrane during subantral augmentation in the group with endoscopic sinus surgery according to the traditional method was 21.21%, which is significantly higher than in patients who underwent surgery in accordance with the periosteum preservation principles – 9.59% (p<0.05). The frequency of application of periosteum restore technologies in subantral augmentation was significantly higher in the control group (49.49%) compared with the main group (24.66%) (p<0.05). The frequency of other complications after subantral augmentation in patients who underwent surgery according to the traditional method was 20.59%, which is significantly higher than in patients with periosteum preserving principles – 10.27% (P<0.05). A reduction in the frequency of complications in subantral bone augmentation, provided that the principles of periosteal preservation in sinus surgery are observed, can be associated with low traumatization of the periosteum, and therefore the preservation of its elastic features with the avoidance of tight adhesions between the periosteum in the area of elevation and the underlying bone.

LEGAL PROTECTION FOR PARTIES IN THE IMPLEMENTATION OF SUBROGATION AGREEMENTS
Journal ID : AMJ-19-07-2022-10306; Author : Dewi Padusi Daeng Muri, Erlyn Indarti, Budi Santoso,
Abstract : In certain nations, surrogacy helps couples conceive. Surrogacy is still illegal and unregulated in Indonesia. Surrogacy is a way for childless couples to escape divorce. Surrogacy is regarded as more successful than other reproductive procedures. This paper tries to answer how the policy of legal protection for the fulfilment of couples' rights to obtain offspring with a surrogacy agreement; and how the implementation of a surrogacy agreement in the context of legislative reform in Indonesia can ensure legal certainty. This study is socio-legal. Werner Menski's Legal Pluralism theory is Constructivist-based. Discussion results show numerous crucial points: First, legal protection is a state of law's subjection to its inhabitants' humanity. A law must contain fairness, certainty, and expediency. Married couples who do not have children can be protected by law in Indonesia so that they can have offspring through a surrogacy agreement using a surrogate mother with the legal policy that if the wife has a uterine abnormality, her uterus was removed due to treatment for other diseases or has heart or lung disease that can harm the pregnant woman. Second, regarding the implementation of a surrogacy agreement in the context of legal reform in Indonesia with comparative studies of law in other countries regarding surrogacy, the authors offer only one type of Gestational surrogacy with the steps starting with building awareness that the purpose of the law is justice, stability, and prosperity.

Journal ID : AMJ-15-07-2022-10305; Author : Kundiy, Zh. P., Dvornik, V.M., Skripnikov, A.M., Gerasimenko, L.O., Kutsenko, N.P., Rud, V.O., Kydon, P.V., Vasylyeva, G. Yu.,
Abstract : On February 24, 2022, Russia sold a full-scale war against Ukraine. Successful acquisition of knowledge and mastering the medical profession in wartime in distance format puts new tasks and challenges to modern students, so the study of the impact of multicomponent stress on medical education applicants, as well as psychological factors and mechanisms that will help to overcome is extremely relevant in nature-combined stress. The study was attended by 106 students of the I-III courses of Poltava State Medical University. Analysis of the study results conducted using the technique of diagnostics of stress resistance and social adaptation by Holmes and Rage, revealed a low level of stress resistance in 36.7±0.3% and the marginal level of stress resistance is 47.2±0.3%. As a research result, a low and marginal stress resistance level were found in the vast students` majority of advanced medical education, which reflected the extremely negative impact of multicomponent stress on the success of studies in general, and the physical and psychological well-being of students. Students with a low degree of stress resistance demonstrated a high degree of stress load, therefore they belong to the risk group of developing psychosomatic disorders and adaptation disorders, they need psychological support in order to optimize the psychological resources of the individual to successfully cope with the stress state, minimize its negative impact, form sustainable ways of protective behavior and overcoming , which in turn will be a guarantee of preserving the health of future health care system employees.

“Association of Perineural Invasion with Depth of Invasion and Neck Node Metastases in patients with Squamous Cell Carcinoma of Oral cavity.”
Journal ID : AMJ-12-07-2022-10302; Author : Dr Lucy Pattanayak1,Department of Radiation Oncology, Acharya Harihar Post Graduate Institute of Cancer, Cuttack, India;, Dr Manoj Kumar Dash2,Department of Community medicine, PRM Government Medical College, Baripada, Mayurbhanj, India;, Dr Paresh Kumar Behera3, Department of Head & Neck Oncology, Acharya Harihar Post Graduate Institute of Cancer, Cuttack, India;, Dr Abani Kanta Nanda1,Department of Radiation Oncology, Acharya Harihar Post Graduate Institute of Cancer, Cuttack, India;,
Abstract : The aim of this study was to evaluate the association of perineural invasion (PNI) with a depth of invasion (DOI) and neck node metastasis in patients with squamous cell carcinoma (SCC) of the oral cavity. A total of 201 eligible patients with histopathologically confirmed SCC of the oral cavity were enrolled. Patients were analyzed in two arms, Arm A (PNI-positive) and Arm B (PNI-negative). The clinicopathological profile along with the association of DOI and neck node metastases with PNI were studied in both arms. All data were analyzed using SPSS version 22. Among 201 patients, the majority were from carcinoma of the buccal mucosa (91,45.3%), followed by the oral tongue (59,29.4%). Histopathological evidence of PNI was present in 107 (53.2%), DOI of more than 4 mm was seen in 165 (82.1%), and less than or equal to 4 mm was seen in 36 (17.9%) patients. Out of 102 PNI-positive patients, 64 (62.7%) had positive neck node metastasis, whereas out of 89 PNI-negative patients, 39 (43.8%) had positive neck node metastasis. This association of PNI with neck node metastases was found to be significant (p = 0.013). Here, 10 patients had unknown nodal status. The association of PNI with DOI was found to be statistically significant (p = 0.00). Out of 165 patients with DOI greater than 4 mm, 101 (61.2%) were PNI positive, while out of 36 patients with DOI less than equal to 4 mm, only 6 (16.6%) were PNI positive. PNI was also found to be associated with lymphovascular space invasion (p = 0.00) or extracapsular extension (p = 0.001) and was statistically significant. The presence of PNI in oral cavity cancers is found to be strongly associated with DOI and neck node metastases, suggesting that the presence of PNI implies tumor biology aggressiveness.

Abstract : To study the clinical profile, incidence, and pattern of congenital coronary artery anomalies (CAAs) in adults undergoing coronary angiography. Coronary angiogram of 3600 patients done between September 2017 and September 2021 were reviewed for the presence of CAAs. Angiography data of 3600 patients done between September 2017 to September 2021 was reviewed in the present study and 88 patients were found to have CAAs giving a prevalence of 2.44%. Mean age of patients was found to be 56.86±11.49 years (range 24-75 years). Among these, the anomalous origin and course of the coronaries were the most common anomaly seen in 73 (2.02%) patients, followed by intrinsic anomalies of the coronary arterial system in 13 (0.36%) patients, followed by anomalous coronary termination in 01 patient (0.03%) and single coronary artery 01 patient (0.03%). Overall, the anomalous origin of the right coronary artery (RCA), with RCA arising from left coronary sinus was the most common anomaly seen in 23 patients (0.64%) followed by absence of the left main trunk with a separate origin of the left anterior descending (LAD) and left circumflex artery (LCX) seen in 20 (0.56%) patients. Posterior origin of RCA within right coronary sinus was seen in 13 (0.36%) patients. There were 6 cases of high origin of right coronary artery (RCA) (0.16%). There were 10 patients (0.28%) of LCX arising from right coronary sinus and 01 case of LMCA arising from right coronary sinus. Split RCA was seen in 09 (0.25%) patients whereas split LAD was seen only in 04 (0.11%) patients. Further, there was only 01 (0.03%) patient of anomalous coronary termination (RCA to right atrium fistula) and 01 (0.03%) patient of single coronary artery. The prevalence of congenital coronary anomalies in the study was found to be 2.44%. and commonest anomaly was that of origin and course of the vessels (88%). Though the prevalence of CAAs in this study was similar to that in previous studies, the pattern of anomalies was slightly different from previous studies.

Journal ID : AMJ-11-07-2022-10300; Author : Vikas BR, Raghav GN, Shreyas Kumar,
Abstract : Chronic kidney diseases is associated with disorders of structure and functioning of thyroid gland. Metabolism and hormonal levels may vary in patients having chronic renal failure (CRF). The objective of the study is to analyse the biochemical abnormalities of thyroid function tests in CRF and to correlate its severity and alterations of thyroid indices. Blood samples were analysed from patients (N=152) for serum triiodothyronine (TT3), serum thyroxine (TT4), serum free triiodothyronine (FT3) serum free thyroxin (FT4), thyroid stimulating hormone (TSH). Blood urea estimation was done using diacetyl monoxime (DAM) method and serum creatinine estimation by Modified Kinetic Jaffe method. Clinical examination and history was performed considering thyroid and renal diseases. Mean age of patients was 53.93 ± 9.91 years of which 115 (75.66%) were males and 37 (24.34%) were females. The mean value of TT3 in CRF stage 3, 4, 5 were 118.38±16.66, 111.26±23.13, 85.35±28.77 ng/mL respectively (p= < 0.00). The mean value of FT3 in CRF stage 3, 4, 5 were 2.84±0.43, 2.41±0.58, 2.07±0.66 pg/mL respectively (p=<0.01). The mean value of TT4 in CRF stage 3, 4, 5 were 1.22±0.17, 1.05±0.21, 0.96±0.27 respectively (p= < 0.001). The mean value of FT4 in CRF stage 3, 4, 5 were 1.19±0.00, 1.04±0.24, 0.98±0.24 pg/dL respectively (p= <0.001). TT3, TT4 and FT3 were found to decrease progressively as the stage of CRF increased. Thyroid hormone abnormalities could be a risk factor for cardiovascular disease and kidney disease progression.
