Management of Lipoprotein (a): A Mini Review

Abstract : Lipoprotein (a) [Lp[a]] is a particle containing apolipoprotein bind to LDL cholesterol. Lp(a) encoded by LPA gene. Independent from LDL-C level, raised Lpa level by genetic studies shows increasing risk of cardiovascular disease. Prior study shows most of patient with LDL-C controlled by statin still have raised Lp(a) level. Current therapy to lowering LDL-C, could not achieve expected lowered Lp(a) level. Genetic therapy as a novel treatment for lowering Lp(a) is still under investigation. We review several study about Lp(a) correlation with LDL-C, coronary disease, and novel treatment. Most of genetic therapy give promising effect although still under early clinical trial. Prior lipid lowering treatment did not achieve as high as genetic therapy to lowering Lp(a) level.

Spiritual Distraction Therapy on Chronic Joint Pain Among Elderly in Coastal Area

Abstract : This aim research was to determine the effect of spiritual distraction therapy in reducing chronic joint pain among elderly living in coastal area. This study was quantitative with quasi-experiment and pre-post-test control group design. The number of respondents was 70. Spiritual distraction therapy was given during seven days. The data collection used observation sheet which contain PQRST pain assessment and the VAS pain scale. Statistical test using the Wilcoxon and Mann-Whitney tests. The study results of the Wilcoxon rank test showed that there was a significance difference in pain area (p-value<0.000) and the time of pain (p-value <0.000) in the treatment group. The Mann Whitney test results showed that there was a significance difference between control and treatment group in pain area (p-value=0.038) and the time of pain (p-value=0.45). The spiritual distraction therapy was effective in reducing the pain area and time of pain occurrence of joint pain in the elderly who live in coastal area. Further study needs to explore the effect of spiritual distraction on pain in other population.

Detection of the Level of IgM and IgG Antibodies in the Serum of Aborted and Non-Abortive Women in Diyala

Abstract : The current study was conducted in Diyala Governorate during the period from 20/4/2021 to 20/8/2021 on women who had aborted and non- abortive women. A questionnaire was prepared to collect information on each case included in the study. The examination was conducted on 250 serum samples using a latex test, the highest abortion rate was recorded in the age group 26-30 years, with a rate of 31.57%. There are significant differences at the level of P≤0.05. The incidence was higher in the city, at 64.63% and the highest rate was recorded in the middle-income group, at 54.87%, and it was higher among housewives, at 75.60%. It was also noted that the infection rate was higher among women who suffered from first period miscarriage, 46.34%. Either the abortion stage was higher during the trimester the first pregnancy a rate of 60.97%. There are significant differences at the level of P≤0.05. Nine cases of IgM positive for aborted women were recorded out of 250 cases, at a rate of 3.44%, and the highest percentage appeared in the age group of 21-25 years, and it amounted to 5.71%. The highest rate of infection was in the age group 26-30 years, amounting to 31.57%. 5 cases were recorded positive for both IgM + IgG antibodies out of 250 cases, at a rate of 1.88%, and the highest rate of abortion was recorded in the age group 21-25 years, which amounted to 2.85%. The study showed a high percentage of IgG antibody compared to IgM antibody ratios for both abortions when the diagnosis was made by latex test.

Knowledge of and attitudes toward eating disorders among psychiatry residents

Abstract : Eating disorders (EDs) are associated with a significant risk of comorbid psychiatric conditions and medical complications along with high mortality rate and suicide risk. This study aimed to assess psychiatry residents’ knowledge and attitudes toward eating disorders. This descriptive cross-sectional study was conducted among psychiatric residents in Saudi Arabia. A self-administered questionnaire was distributed electronically to psychiatric residents through the Saudi Commission for Health Specialties database. The questionnaire collected demographic information (e.g., age, sex, and nationality), previous experience with eating disorders, and assessment of knowledge of and attitudes toward eating disorders. Fifty-two psychiatric residents (65.4% men, 34.6% women) were included in this study. Most participants (65.4%) belonged to the 20–30-year age group, and 46.2% of all participants were service residents. Knowledge of eating disorders was poor among 63.5% of residents and good among 36.5% of residents. There was a positive attitude toward eating disorders among 76.9% of residents, while 21.3% showed a negative attitude. The younger age group (age <35 years) showed significantly better knowledge, Saudi nationals, training residents, and those with ≤4 years in psychiatric practice; however, there was no difference in their attitudes. While psychiatric residents had an optimistic attitude toward eating disorders, they lacked knowledge of eating disorders. Among all participants, young Saudi trainee residents who were at an early stage of their career had better knowledge than those in the other groups.

Application of Spatial Analysis to Nutritional Epidemiology among Under - Five Children in Karnataka, India

Abstract : Malnutrition has become a significant public health challenge strongly associated with the substantial increase in the risk of mortality and morbidity in children. This study will be conducted to determine the geographical variation in nutritional status of under-five children and identify the spatial clusters based on demographic and household characteristics in Karkala taluk of Karnataka state, India, and replicate the same in other areas. The aggregated nutrition status data of children under five years are collected from the Udupi district health office for three years, validated at the village level. The anthropometry, topography, and socio-demographic data is collected by visiting the household. The retrospective discrete Poisson probability model will identify the clusters of underweight, stunting and wasting. Spatial autocorrelation statistics like Local Indicators of Spatial Association (LISA) statistics of nutritional status will be analysed. The logistic regression model will be used to analyse the causes of clustering. The geo-coordinates of the households were summarised using a Cartesian coordinate system. The SaTScan statistic software (SaTScan v10.0.2) will be used to identify the spatial, Spatio-temporal clusters of underweight, stunting and wasting among under-five children. This software will detect the randomness of nutritional status of under-five children over space and space-time. The spatial data analysis and spatial autocorrelation will be carried out using GeoDa software. Spatial dependence of nutritional status will be analysed through the Global Moran's Index and Local Indicators of Spatial Association (LISA). The observed and expected cases inside the window are considered for spatial and space-time cluster significance in SaTScan software. The LISA statistics will indicate the local pockets of non-randomness or hot spots. The LISA statistic is also used to assess the contribution of location on the magnitude of global figures. The Spatiotemporal and autocorrelation results can be used for risk analysis of malnutrition among under-five children by the policymaker for decision making.

Evaluation Alcoholic Extract Effect of Vitex agnuscastus on the Testes Histological Structure of Adult White Rabbits Orectolagus cuniculus

Abstract : This paper aims to evaluate the effect of the alcoholic extract of the Vitex agnus castus on the histological structure of the testes of adult White rabbits. In this study, 12 White male rabbits were used, which were divided into two groups, The first being the control group and its number was 4 rabbits, and the second group included 8 rabbits, and this group in turn was divided into two groups with 4 rabbits for each group. The two groups of rabbits were injected with the alcoholic extract of the Vitex agnus castus at a concentration of 100 and 200 mg of extract/kg of body weight. One concentration per group is injected for 30th days. At the 30th days, the treated animals was sacrificed and the testes were removed from them, and then their tissues sections were prepared. The results obtained from this study showed that all the animals of the two groups of experiments that were treated with the extract showed a change in the thickness of the walls of the seminiferous tubes in the testes and their appearance became undulating and irregular. Also it was observed that some germ cells were Shed and depleted and collected in the lumen of seminiferous tubes and the appearance of Phagocytic cells inside the cavity and congeston of blood vessels. This study concludes that Vitex agnus castus can reduced the number of sperms in the testes and resulting in damage to the testes which the regularly usage can lead to infertility.

Teratogenic effect of hydroxychloroquine on the histological structure of adult rabbit Kidneys Orectolagus cuniculus

Abstract : Hydroxychloroquine (HCQ) is an effective drug for the treatment of various autoimmune disorders. It has appropriate pharmacological properties that show its effectiveness in improving the control of parasitic infections such as malaria. It is rapidly absorbed into the plasma and excreted through the kidneys. This study was designed to investigate the effect of hydroxychloroquine (HCQ) at two concentrations of 25 and 50 mg/kg/day on the histological structure of the kidney in an adult rabbit (Orectolagus cuniculus). The study was conducted on 18 rabbits, it was divided randomly and equally to three groups included a control group and a group that was treated with a concentration of 25 mg/kg/day of the drug, while the animals of the third group was injected with a dose of 50 mg/kg/day. The results of the histological study showed the occurrence of pathological histological changes to the kidneys in the group of animals treated with the drug of 25 and 50 mg/kg/day concentration, the most prominent of which was a clear congestion of blood in some blood vessels in the cortex area and between the epithelial cells lining the glomeruli. It also showed a clear cellular infiltration near the blood vessel. Degeneration of the lining of the epithelial cells of some renal tubules, especially the proximal convoluted tubules, and the separation of tubule cells from the detached basement membrane and their gathering in the lumen of the tubules in most tissue sections was observed. It was also found that the glomerulus was enlarged and that treatment with this drug led to a decrease in Bowman’s space, in addition to an increase in the thickness and density of the basement membrane of the urinary tubules. Glomerular cells suffered from accumulation, shrinking, enlargement capsular space, and cell death. It can conclude from the above results that the hydroxychloroquine drug has clear effects on the histological structure of the kidneys.

Psychotropic Medication Effects, Management, and Treatment of Sexual Dysfunction: a Systematic Review

Abstract : This study aimed to systematically assess the impact of psychotropic medication-induced sexual dysfunction on all sexes and to outline the safety of relevant medical interventions. Several databases were searched using key terms to derive pertinent data across randomly selected population groups. Viable literature was screened using eligibility criteria, selecting the most vital and reliable information. Conflicting information and heterogeneity across studies required a systematic narrative to represent the included data. A bias risk summary was calculated using the Cochrane Tool for Risk of Bias calculation. Our search yielded more than 2000 eligible studies. Further stratification was performed to eliminate non-pertinent data, and characteristics of selected studies were tabulated. Selective serotonin reuptake inhibitors (SSRIs) and typical antipsychotics were strongly associated with sexual dysfunction across various study populations. Switching from SSRIs to serotonin-norepinephrine reuptake inhibitors and/or switching from prolactin-raising antipsychotic psychotropic medication was associated with improved sexual function. Pharmacological interventions were suggested by a majority of the included studies. Non-pharmacological interventions were associated with improved sexual function across all sexes. Several extracts, including maca root, saffron, and rosa damascene oil, were associated with improved sexual function.

Immunomodulatory and Anti-Inflammatory Effect of Mentha pulegium L

Abstract : Used of Immunomodulator and anti-inflammatory drug is a new application in the treatment disease. The most immunomodulator and anti-inflammatory was from medicinal plants. Mentha pulegium L (MTH) is traditional medicinal plant widely used in the world, in this study investigated the Immunomodulatory and anti-inflammatory activity of MTH leaves ethanol extract and its fractions with the effect of angiogenesis cytokine gene expression and production. Macrophage RAW 264.7 cell was used to investigate the Immunomodulatory and anti-inflammatory effect both MTH leaves extract and fractions. The results were showed significant increase in the viability of treated RAW 264.7 cell with MTH leaves extract and fraction1 and inhibited Nitric oxide production, while enhancement the production of angiogenesis cytokine and increased intracellular production of INF-  , IL6 and IL8 in dose dependent manner. In conclusion of the outcome the M. pulegium L leaves extract be as new Immunomodulation and anti-inflammatorydrug.

Risk factors of mortality among patients with Coronary Artery Bypass Grafting attending Cardiac Center of Erbil City; A cross-sectional study.

Abstract : Coronary artery bypass grafting (CABG) is a major surgical operation where atheromatous blockages in a patient’s coronary arteries are bypassed with harvested venous or arterial conduits. Post-CABG mortality has been associated with multiple patient factors such as advanced age, preoperative renal dysfunction, diabetes, low preoperative Ejection Fraction (EF), stroke, carotid artery disease, dysrhythmias, concomitant valvular disease and multiple other comorbidities. To assess the mortality rate and discuss the possible risk factors associated with mortality following CABG. A cross sectional study was conducted in the Cardiac Center in Erbil, Kurdistan region of Iraq from June 2020 to March 2021. Data was collected from case files of 200 patients in the hospital records and follow up data was obtained after a 6-15-month period by interviewing the patients directly. Data were analysed using the Statistical Package for Social Sciences version 23 (SPSS, IBM, Armonk, NY, USA). The mortality rate after CABG was 8.5% in the study duration, with the majority being in the 50-59 age group. Female gender, left ventricular dysfunction, hyperlipidemia, double vessel disease and triple vessel disease, valvular disease and concomitant valve surgery were identified as risk factors for mortality in the postoperative period. More efforts should be directed towards preventing and controlling cardiovascular risk factors to decrease the prevalence and impact of CAD in our community.
