Protective Effects of Sufentanil and Remifentanil Preconditioning on Testicular Ischemia Reperfusion Injury

Abstract : Testicular torsion is a condition that manifests with acute pain in childhood and adolescence which can lead to infertility despite urgent surgical intervention. The aim of this study is to investigate and compare the protective and preventive effects of sufentanil, remifentanil and fentanyl, a potent analgesic agent on testicular ischemia-reperfusion injury (tIR). 28 adult male Wistar rat were divided into four groups (n=7). Torsion was performed on all left testicles rat 720' clockwise. 1 hour ischemia. 30 minutes before detortion, control group (Nacl 0.9%), fentanyl group (2 mcg/kg), Remifentanil group (1 mcg/kg), Sufentanil group (0.6 mcg/kg) were injected intraperitoneally. During 1 hour of ischemia, the left testes was detorted to restore physiology. 24 hours after reperfusion, rats in euthanasia were taken blood to see Malondialdehyde levels and ipsilateral testes to find germ cell damage. Leydig cells count and percentage tubular necrosis. Administration of Sufentanil before detortion significantly reduced malondialdehyde levels (6.01+0.24SD,P<0.05), damage to germ cells (5.93,P<0.05), Leydig cells count (20.2+4.5SD,P<0.05) and tubular necrosis (3.95+0.62SD,P<0.05) which were subjected to torsion compared to the fentanyl and remifentanil groups. However, lipid peroxidation content of malondialdehyde was significantly decline in remifentanil group (3.58+0.23SD,P<0.05). Sufentanil in ischemia preconditioning has the most protective effect compare to remifentanil by preventing testicular ischemia reperfusion injury leading to infertility in the future.

Effect of intraarticular hyaluronic acid injection on knee OA patients

Abstract : Osteoarthritis (OA) is a shared progressive disorder in old-aged adults that is marked by joint pains and dysfunctions. Hyaluronic acid (HA) is an integral constituent of synovial fluids that play as joints lubricants throughout shear-stress and shocks absorption Intraarticular injecting by exogenous HA is proposed to substitute this OA-persuaded deficit and for production stimulation of endogenous HA, which can alleviate signs of knee OA by means of various path-ways. Determine the impact of intra-articular HA injections on knee Osteoarthritis cases. A clinical trial to assess the impact of intraarticular HA on gait functions and clinical outcomes detected in radiographic imaging moderate-severe knee OA cases. A sample of 100 patients from 40 to 80-yrs old, provided knowledgeable agreement at the pretreatment visits 1-wk previous to the 1st injection, and all contributors have detected knee OA built on the criteria set forth via the American College of Rheumatology. A highly significant change was found among different periods regarding WOMAC score. We concluded that the findings of this work showed that a regime of intraarticular injections of hyaluronic acid has a significantly high influence on functional outcomes and a decrease in osteoarthritic knee pains for at least six months.

Detection of extended spectrum beta lactamase producing Proteus mirabilis by phenotypic and genotypic methods among various clinical isolates

Abstract : Despite the advent of various antimicrobials, nowadays, the community acquired and health care associated infections are relatively frequent with Proteus species. Its resistance to various groups of antibiotic has been increasing, which needs a continuous survey for better therapeutic response. From total 5055 samples, 591 (11.7%) Proteus mirabilis isolates were obtained. 186 (5.4%) from urine samples, 120 (18.5%) from wound 105 (30.7%) from blood, 172 (28.5%) from pus were isolated. Out 591 samples of Proteus mirabilis 185 (31.3%) were ESBL positive. Maximum samples were obtained from wound (37.5%), followed by pus (36.0%), urine (31.7%), and blood (16%). Statistical analysis was done using chi – square test and p value was found to be significant. The frequency of ESBL producing strains in our study was 31.3 %. In the present study, presence of ESBLs was determined with CLSI phenotypic confirmatory test (PCT), double disc diffusion test (DDST) and E-test. PCT detected 185 (31.3%) isolates, DDST detected 169 (28.5%) isolates and E - test detected 185 (31.3%) isolates. Thus a higher rate of ESBL detection was found with CLSI phenotypic confirmatory test. All the positive isolates detected in PCT were confirmed by MIC using E-test. The clinical laboratory standards institute (CLSI) also recommends the use of the combination disc method for the phenotypic confirmation of ESBL production among Enterobacteriaceae. The raising resistance to various groups of antibiotics is a common problem and its management is a subject of concern. Identification, surveillance and study of the epidemiology of antimicrobial resistance will assist in the therapeutic management of patients by reducing the prescription of large spectrum antibiotics control of infections. This study is therefore a step towards the generation of national data on the prevalence of antimicrobial resistance pathogens.

Comparing the probable complications and hemodynamic changes in Electro Convulsive Therapy under anesthesia with two 0.3 and 0.6 doses of Succinylcholine

Abstract : Electroconvulsive therapy (ECT) is one of the oldest and most effective methods to treat many psychiatric disorders. Succinylcholine is a short-acting depolarizing skeletal muscle relaxant used to relax muscles and facilitate patient control during intubation, mechanical ventilation, and surgical procedures. The aim of this study was to compare the probable complications and hemodynamic changes created as a result of applying 0.3 and 0.6 doses of Succinylcholine in candidates of ECT. In this clinical trial at Isfahan university hospital, 40 ECT candidate patients were randomly divided into two groups of 20 patients receiving 0.3 and 0.6 Milligrams per Kilogram dose of Succinylcholine. The age, sex, height, weight, and body mass index (BMI) of patients were recorded. Before induction of anesthesia (baseline) and 1, 5, 10, and 20 minutes later, symptoms like systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR) and oxygen saturation (SPO2) measured and compared between two groups. There were no significant differences between the two groups regarding primary data and symptom values (P> 0.05). In patients that received 0.6 mg/kg of Succinylcholine, the prevalence of hypoxia and duration of apnea were significantly higher and the seizure duration was considerably shorter. Administration of Succinylcholine with 0.3 mg/kg lead to fewer complications including frequency and duration of apnea.

Pharmacological and Nutraceuticals Profile of Citrus medica: A Vitamin C Rich Source

Abstract : Citrus medica commonly known as citron is widely distributed in the himalayan regions of India. It contains various phytoconstituents like limonene, hesperetin, Vitamin C, phenolics, flavonones, iso-limonene, pectin, linalool, decanal, and nonanal etc. It is frequently used in traditional systems of medicine for the treatment of cardiac, thyroid, and oestrogen disorders. These activities are due to presence of phenolic and flavanoids content in it. It is also act as mutagenic and exhibits good antioxidant properties. The plant leaves, peels, fruits and their seeds have several pharmacological effects such as antioxidant (reduces reactive oxygen species and malondialdehyde), analgesic and anti-inflammatory activities (inhibitis DPPH and hydrogen peroxide radical scavenging), antilipidaemic (by inhibiting HMG-CoA reductase) and estrogenic activity (increase vaginal epithelial cell cornification and uterine weight) etc. The purpose of this review is to highlight the source, geographical indication, phytochemistry, traditional uses, pharmacological effects and numerous nutraceuticals application of Citrus medica and its main constituent Vitamin C.

The Effect of Positive Thinking on Depression Level in Diabetes Mellitus Patients

Abstract : The effect of positive thinking on the level of depression in diabetic Mellitus patients. Diabetes mellitus is a chronic disease that cannot be cured. It is not contagious but has a high risk of disrupting one's life. This study aims to prove the effect of positive thinking on the level of depression in patients with diabetes mellitus. This type of research is a pre-experimental One-Group Pre-test-posttest Design. The sampling technique is purposive sampling. The number of samples in this study was 34 people. Data collection techniques used Beck Depression Inventory-II (BDI-II). The results of the investigation before treatment were positive thinking depression level in study subjects, mild depression in 12 people (35.3%), depression was 18 (52.29%), and significant depression for 4 people (11.8%). After being given a favorable thinking treatment, the depression level of the study subjects was: normal/not depressed in 1 person (2.9%), mild depression in 17 people (50%), moderate depression in 15 people (44.1%), and significant depression 1 person (2.9%). Paired t-test results: mean before treatment 21.9412, after treatment 19.1765, t = 5.787, and p = 0.000. There is a significant effect of positive thinking on the level of depression in patients with diabetes mellitus. Positive thinking can reduce depression levels in patients with diabetes mellitus.

Conversion Time of Covid-19 RT-PCR Test Among Health Workers

Abstract : One of key sector to combat covid-19 is healthcare, whose worker are put in high risk affected physical, mental and social. Healthcare worker (HCW) defined as all staff involved to provide treatment for covid-19 patients and are put in higher risk to SARS-CoV-2 exposure than general population. In addition, infected HCW are required to work immediately as the amount of HCW is limited. Indonesia applied entry and exit test in five days to shorten isolation and quarantine period. The policy has also been applied to HCW in Sulianti Saroso Infectious Disease Hospital since January 2022. This study accounted 229 covid-19 confirmed healthcare worker in Sulianti Saroso Infectious Disease Hospital with admission date January 1st – February 28th, 2022. Conversion time was divided into two categories, less than equal to 10 days and more than 10 days. Our study showed out of all, only 20,5% of our healthcare workers had conversion ≤10 days. It means most of the result of exit test RT-PCR on day 5 after confirmed date was positive. We found correlation between symptoms and conversion time (p = 0,0001). We also found significant correlation between severity and conversion time (p = 0,0001) as in line to preceding studies. this study aimed to give recommendation and evaluation on exit test on day 5 for healthcare worker policy.

Analysis of Ground Support Equipment (GSE) Failure Modes at PT. Gapura Angkasa Makassar

Abstract : This research aimed to identify and detect each existing failure mode based on probability and severity and provide solutions using the Failure Mode and Effect Analysis (FMEA) method. A company known as PT. Gapura Angkasa in Sultan Hasanuddin Airport was used, which has hundreds of workers in the Ground Support Equipment (GSE) Division, and its operation is fraught with danger and risk. This research identified the cause of the GSE damage, ensuring the safety of workers and the smooth operation of the aircraft and passengers. A qualitative approach was used to examine the condition of the equipment, with the researcher serving as the primary instrument. To obtain information about the failure modes of the GSE at PT. Gapura Angkasa Makassar, the FMEA method was used. This was carried out by determining the risk rating, which is represented by the Risk Priority Number (RPN). The results showed that there were two types of GSE damage, namely BTT and BCL. The engine indicator component in the BTT and the steering column in the BCL had the highest damage, with RPN of 240 and 160, respectively. Routine maintenance is recommended on components with a high probability value to improve their reliability, while special attention should be paid to components with a high RPN level.

Effects of a health education intervention on insecticide treated nets use and its practices among pregnant women attending antenatal clinic in Royal hospital, Enugu State, Nigeria: A Pilot study

Abstract : Malaria remains the most prevalent parasite infection accountable for the high burden of disease and deaths in low-income countries. Approximately half of the global population lives in malaria-endemic areas, pregnant women and young children below the age of 5-years are considered as high-risk group for the transmission of malaria. A pilot study was conducted in Royal hospital, Enugu State, Nigeria. The health education intervention contents were prepared initially and pre-tested by piloting 30 pregnant women. The pre-test was done two times, with an interval of 10 days in order to evaluate the accuracy of the original findings with a retest. A total of 30 pregnant women participated in the study Majority of the pregnant women belong to Igbo ethic group 27(90%). All the pregnant women live in the city 30(100%). The result of this study reported that a total of 29 respondents had good knowledge of malaria prevention practice. Results proved that health education could be an effective if it is included in routine sessions provided to pregnant women in order to encourage them to practice the use of ITN. This could not only be beneficial in preventing malaria and reducing its burden of disease, it can also improve maternal morbidity and mortality among pregnant women.

Effect of Pancreas Omentoplasty and Sleeve Gastrectomy on Body Weight, Blood Sugar Levels, and Interleukin-6 in Diabetic and Obese Rats

Abstract : This study described the effectiveness of pancreas omentoplasty with sleeve gastrectomy on weight reduction, improving blood sugar levels and interleukin-6 in diabetic and obese rats. A randomized control group pretest-posttest study design was performed on 12 male Wistar rats aged 6 weeks induced with obesity and type 2 diabetes mellitus (DM) and grouped into sleeve gastrectomy (SG) group (P1) and SG group with pancreatic omentoplasty (P2). SG is done by partially removing the stomach in the major curvature and suturing it. Omentoplasty is performed by suturing the omentum to the pancreas. Body weight, fasting blood glucose levels, and interleukin-6 (IL-6) were examined before and 10 days after treatment. None of the rats experienced postoperative drop-out, and examination showed that they were obese with homogeneous body weight, blood glucose, and IL-6 levels. Data analysis showed body weight, blood glucose levels, and IL-6 after treatment were significantly decreased in both groups compared to levels before treatment. The improvement of these parameters was found to be significantly better in the group that received sleeve gastrectomy and pancreatic omentoplasty. In conclusion, sleeve gastrectomy with pancreatic omentoplasty significantly reduce body weight, blood sugar levels, and IL-6 better than sleeve gastrectomy procedure alone.
