Multimodal Imaging Diagnosis of Hemophilic Knee Arthropathy: A Case Series

Abstract : Hemophilia is a congenital disease resulting from a severe coagulation factor VIII or factor IX deficiency. This condition causes the emergence of target joints, which in 30% of hemophilic patients develops hemophilic arthropathy. We reported three cases of hemophilic arthropathy that were examined using conventional radiology modalities, ultrasound and 1.5T MRI at Dr. Kariadi Semarang. Conventional radiological examinations were assessed using the Arnold Hiltgartner qualitative scale and the Petterson quantitative score. The first case was an 11 years old child with type B hemophilia, and right knee pain with an Arnold Hiltgartner scale of 4 and a Petterson score of 7. An MRI examination obtained an IPSG MRI score of 7. The second case is an 8 years old child with hemophilia type B who complains of a swollen right knee. On examination, the Arnold Hiltgartner scale was 4, and the Petterson score was 7. The results of the ultrasound examination showed a HEAD-US score of 6. The third case is a 28 years old man with hemophilia type A and right knee pain, with an Arnold Hiltgartner scale of 5 and a Petterson score of 12. In conclusion, conventional radiography is a sensitive initial examination for moderate and severe degrees of hemophilic arthropathy. Ultrasound modality is an effective follow-up examination in assessing soft tissue abnormalities and osteochondral changes, while MRI has the best sensitivity for detecting hemophilic arthropathy.

Electrochemical Synthesis and Characterization of Al2O3 /ZnO Nanocomposite

Abstract : Nanomaterials have excellent chemical and physical properties and have wide applications in the modern time. One of the important methods for preparing nanocomposites (Al2O3/ZnO) is the electrochemical deposition method, which is characterized by speed, ease and inexpensive. The nanocomposite characterizes by Advanced techniques such as x-ray diffraction (XRD), field emission scanning electron microscopy (FESEM), transmission electron microscopy (TEM), ultraviolet-visible (UV-Vis), energy dispersive x-ray (EDX), and atomic force microscopy (AFM). The analysis showed that the shape of nanocomposite was Nano rod, the band gap value was 3.23(e.v) and average crystal size was (8.4 nm).

A Clinical Evaluation of Kim et al. Prediction Scale of Impacted Lower Third Molar Extraction Difficulty

Abstract : Determining the difficulty of impacted lower third molar (L3M) is a continuous challenge for oral surgeons. Kim et al., a modification of Pederson scale is one of the indices proposed to estimate the difficulty of (L3M) based on radiographic findings. Preoperative panoramic view were obtained for fifty patients who need (L3M) being extracted the difficulty was predicted according to Kim et al. A clinical evaluation of difficulty was considered according Parant scale (PS) and surgical time (ST). Kin et al. revealed low to accepted sensitivity (23.8%, 58.8%) and low specificity (21.1%, 36.4%). Regarding the likelihood ratios, only the negative likelihood ratios for prediction of Parant categories were significant (3.61) while the other ratios were not. No significant correlation (P= 0.538, 0.229) exist between the index difficulties with PS and ST. Kim index is unreliable to predict extraction difficulty.

POTENTIAL USE OF FERMENTED DANGKE CHEESE TO IMPROVE GLYCEMIC CONTROL IN RATS FED WITH A HIGH-FAT GLUCOSE DIET AND PROPYLTHIOURACIL

Abstract : Dangke is a local cheese from Enrekang, South Sulawesi, traditionally made with buffalo milk and papaya sap. Dangke is a good source of lactic acid bacteria, which may offer health benefits. This study aimed to investigate the anti-hyperglycemic effects of dangke in rats. In addition, this study also examined the effect of fermentation time on the number of lactic acid bacteria obtained from dangke. Dangke was prepared by boiling buffalo milk and papaya sap until solidified and fermented in a jar for 1, 3, 7, and 14 days at room temperature. The number of lactic acid bacteria from dangke was determined using a Total Plate Count method. Hyperglycemia in rats was induced with propylthiouracil, a high-fat diet, and dextrose 10% in drinking water for 14 days, followed by dangke cheese (0.5 or 1.5 gram per 200 grams of rats’ body weight) treatments for another 14 days along with high-fat diet and dextrose. It was found that the lactic acid bacteria counts obtained from dangke after 1, 3, 7 and 14 days of fermentation were 0.62 x 108 CFU/mL, 0.912 x 108 CFU/mL, 0.263 x 108 CFU/mL, 0.14 x 108 CFU/mL, respectively. The administration of dangke at the dose of 1.5 grams showed the highest percentage of blood glucose reduction among the treatment groups (p<0.05). In conclusion, three days of fermentation of dangke cheese produced the largest amount of lactic acid bacteria, and a 1.5 g dose of dangke cheese is required to effectively reduce blood glucose in hyperglycemic rats.

Effects of Pumpkin (Cucurbita moschata) Seed Capsule Provision on Total Cholesterol, LDL, and HDL Levels in Patients with Hypercholesterolemia in Palu City: An Experimental Study

Abstract : Hypercholesterolemia is a condition with increased total and LDL cholesterol concentrations, and pumpkin seed extract can be used as a functional food to reduce this complication. The flavonoids, phytosterols, fiber, and antioxidants in pumpkin seed decrease the total cholesterol and LDL levels while increasing HDL. This study aimed to analyze the effect of pumpkin (Cucurbita moschata) seed capsule compared to simvastatin on total cholesterol, LDL, and HDL levels in hypercholesterolemic patients. A quasi-experimental method was adopted with a non-randomized control group pretest-posttest design. A total of 61 respondents were divided into the intervention and control groups. The collected data were analyzed by univariate and bivariate analysis. The results showed a decrease in total cholesterol and LDL levels and a significant increase in HDL in the control group, which was given simvastatin with a p-value of 0.000 each. This was also reported in the intervention group, which was given pumpkin seed capsules with p-values of 0.000, 0.002, and 0.000. However, the decrease in total cholesterol and LDL and the increase in HDL were more common in the control group. Pumpkin seed capsule can significantly reduce total and LDL cholesterol levels and increase HDL, but the reduction is lower than simvastatin.

Efforts to Improve The Lorong Community Empowerment Model: Management-Based Intervention Model

Abstract : The Healthy Cities Movement in Indonesia has been carried out, although it has been officially effective since the issue of a Joint Regulation between the Ministry of Home Affairs and the Ministry of Health in 2005. Lorong (alley) are the elements that need attention in realizing healthy cities because of their very complex issues, including cleanliness, awareness, and inadequate infrastructure. The involvement of various community groups from the lower level is still not maximized in several areas. There is a need for an appropriate lorong community empowerment model. This study aims to improve the model of community empowerment in the lorong to create a healthy city in Makassar with management-based interventions. The study designs used descriptive qualitative design with 30 Informants. Based on the results of interviews with various agencies, it was found that the conditions of the alley people in Makassar City varied. Some report had knowledge regarding how to protect the environment but did not implement it. Healthy lorong should be able to provide a sense of security and comfort to the people. An informal approach through the door-to-door method is one of the keys in empowering the lorong community. The challenges in realizing healthy alleys are the level of public education and awareness regarding environmental health.

Endorphin Massage Module Development as an Effort to Increase Breast Milk Expenditure in Postpartum Mothers

Abstract : This study aimed to determine (1) the feasibility of the Endorphin Massage module as a learning media (2) the effectiveness of Endorphin Massage module in increasing midwives' knowledge about Endorphin Massage. The method used in this study was Combine Method between Research and Development (RnD) with the Pre-Experimental One Group Pretest-Posttest. Sampling was carried out using the Purposive Sampling Technique which were 40 samples. The data were analyzed using statistical analysis through Wilcoxon Sign Rank Test. The validation test results from media experts were 90.9% in average and the validation results from material experts have an average of 92.7% which indicate that the Endorphin Massage module is very feasible to use without revision. User validation results (midwives) in a small group which consisted of 10 midwives with an average eligibility value by 89.5% and large-scale user validation results in 30 midwives with an average eligibility value by 93.4% showed that Endorphin Massage module is very feasible to use. The results of the effectiveness of using Endorphin Massage module found that the average pretest score (before intervention) was 63.6 in the sufficient knowledge category and the average posttest score (after intervention) was 77.6 in the high knowledge category. Based on the analysis of the pretest and posttest results, there was an increase in midwives' knowledge about Endorphin Massage before and after the intervention. There is a significant difference between the midwives’ pretest and posttest values. Endorphin Massage module is feasible and effective in increasing Midwives’ Knowledge about Endorphin Massage.

MEDROXYPROGESTERONE ACETATE EFFECT ON MMPs AND TIMPs EXPRESSION AS CORNEAL ALKALI BURN THERAPY

Abstract : Ocular chemical burn is one of emergencies in ophthalmology. Chemical burn caused by alkali can cause more damage because it can penetrate the ocular surface very quickly, causing a saponification reaction due to fatty acids in the cell membrane, thereby damaging the corneal and conjunctival epithelial tissues. Medroxyprogesterone acetate (MPA) is a progesterone derivative which properties and structure are similar to steroids and often used as a contraceptive as a hormone replacement at an affordable price. MPA inhibits IL-1β synthesis which induces collagen degradation by corneal fibroblasts and this effect is associated with suppression of expression or activation of matrix metalloproteinases (MMP-1, MMP-2, MMP-3, and MMP-9), as well as tissue inhibitors of metalloproteinases (TIMP-1 and TIMP-2). Previous studies reported that topical or parenteral MPA reduced the incidence of perforation and ulcers in rabbits. MPA has an additional effect as an anti-inflammatory, with fewer complications when compared to steroids. In this review we discuss the molecular effects of MPA as adjuvant therapy in corneal alkali burn.

Assessment of Mental Well-Being Among Iraqi and Arab Board Trainees in Medical city Teaching Center

Abstract : Doctors are not protected from the occurrence and consequences of mental illnesses, doctors (especially board trainees) face particular challenges such as high patients’ attendance, long duty hours, potentially violent situations, and critical decision-making that place them at more risk of anxiety, depression and other stress related psychosocial problem. The current study aimed to identify the prevalence of mental health problem and depressive syndrome among Iraqi and Arab board trainees in Medical City Teaching Center. A cross sectional study was carried out from 1/1/2022 to 1/4/2022 at the Medical City Teaching Campus using a questionnaire consisting of socio-demographic variables, Self-reporting questionnaire (SRQ-20) to detect the presence of mental illness and screened for depression by a standardized scale; Diagnostic and Statistical Manual Version IV (DSM-IV). Among the 535 participants 500 complete and returned the questionnaire with response rate of 93.4%. The prevalence of mental illness was 71.2%, it was more among males 55.9%, single 77.8%, had (≥4 children) 72.7% and trainees of surgical branches 78%. The associations were significant with branch of trainees. The prevalence of depressive symptoms was 15.4%. Higher rates were among males 63.6%, single trainees 35.3%, with children 12% and working in surgical branches 13.7% The associations were significant with branch of trainees. The current study revealed that mental illness is prevalent among board trainees. Male gender, single, having children and working in the surgical specialty were associated with higher rates of both mental illness and depression.

An Investigation of Fatigue among Jordanian Nurses Based on Their Gender Measured Using the OFER 15: A Primary Quantitative Study

Abstract : This study aims to investigate the levels of fatigue among Jordanian nurses. Also, investigate the impact of the nurses' socio-demographic variables on their fatigue levels. Data was collected from a sample of two hospitals in Jordan using a simple random sampling technique. The study was developed through collecting primary quantitative data. Data was collected through closed-ended questionnaires based on the OFER 15 tool as a basis for measuring fatigue. The findings relied on a total of 517 respondents (154 male and 363 female). This was an equitable representation of both genders in the Jordan nurses' workforce. The socio-demographics showed no significant variance among genders apart from smoking and marital status. Using the OFER 15 tools manual and scoring, the findings established a high chronic (Female =90.67%, Male = 76.72%) and acute (Female=77.56%, Male =74.74%) fatigue for female nurses as compared to their male peers. Significant correlations were found between the three types of OFER 15 scores (chronic, acute, and inter-shift-recovery) and five independent variables; Age (chronic), Gender (acute and inter-shift), education level (inter-shift recovery), Marital status (acute fatigue) number of dependents (chronic fatigue) and the weekly working hours (chronic fatigue) levels, respectively. On gender differences, it established that female nurses had a higher chronic and acute fatigue than male peers. However, they also had a higher inter-shift recovery index than the male nurses. There is a high fatigue prevalence among Jordanian nurses. There is an urgent need to address acute and chronic fatigue types as a matter of priority. Providing better resting areas and exercise programs for nurses is highly recommended.
