Ultrasound measured Gastric Cross-Sectional Area in Critically-ill Children

Abstract : Monitoring gastric residual volume (GRV) and holding or interrupting enteral nutrition for large or elevated GRV has had a firm place in the recommendations of critical care nutrition guidelines. The present study aimed to assess the feasibility of measurement and the influence of critical illness on ultrasound-measured gastric CSA. 46 mechanically ventilated patients and 30 older adults were included in this prospective trial. All patients were subjected to careful history taking, thorough clinical assessment, and standard laboratory investigations. In all patients, CSA was measured by ultrasound at the first 24h of admission and compared to age and sex-matched healthy controls. Patients had significantly larger CSA in comparison to controls [median (IQR): 1.1 (0.9 to 1.5) versus 0.7 (0.5 to 1.0)]. Patients with septic shock diagnosis had the highest gastric CSA. At a cut-off of 0.77, CSA has good predictive power for critical illness [AUC (95% CI): 0.72 (0.56 – 0.88)] with a sensitivity and specificity of 81 % and 60 %. The current study suggested gastric size is significantly affected in critically ill children and this needs to be considered when deciding when to start enteral nutrition.

Analysis of the effects of laparoscopic sleeve gastrectomy on weight loss and co-morbidities

Abstract : The sleeve gastrectomy was initially developed as the initial step in the gastric bypass procedure, it is now being carried out on its own an increasing amount. As a result, it is essential to report results on the reduction in the number of co-morbidities as well as weight loss. A continuous group of 80 patients diagnosed with morbid obesity were examined. The operative variables, the number of complications, the amount of weight loss, and the requirement for medication to treat co-morbidity at least six months after surgery were the parameters. The median amount of time spent operating a machine has come down to 74 minutes. The typical length of stay in the hospital was three days. The duration of the follow-up, on average, was 9 months. The typical amount of excess weight lost was 45.5 percent of total body weight. The average number of points off of one's BMI was 24.4 percent. Three quarters of the patients were able to reduce the amount of medication they were taking for diabetes, hypertension, and high cholesterol, or stop taking it altogether. The findings of this study demonstrated that the laparoscopic sleeve gastrectomy is a technique that is beneficial in assisting patients in achieving their weight loss goals. The reduction or cessation of medication in seventy-five percent of the patients was clinically significant.

Preparation, Identification and Characterization of Novel Schiff base Derived from Diamine and Aldehyde Derivatives and its Zn, Co, and Ni Metal Complexes

Abstract : Schiff bases and metal complexes have brought the attention of the researchers due to its wide application as bioactive and anticancer compounds. To synthesis new Schiff base derived from aldehyde and diamine derivatives and its three metal complexes (Zn, Co, Ni). The white crystalline Schiff base (1E,1'E)-N,N'-(ethane-1,2-diyl)bis(1-(4-chlorophenyl)methanimine) (E4CPMA) was synthesized from the reflux of one moles of ethane,1-2-diamine and 2 moles of 4-chlorobenzaldehyde. Te metal complexes were also prepared by the reaction of the metals salts solution with the solution of the Schiff base, the characterization of the new compounds was carried out via CHN, FTIR, UV-Vis, molar conductivity, magnetic susceptibility, and melting point. The physical properties of the new compounds show different appearance and melting point, and the metal complexes shows diamagnetic for zinc and paramagnetic for cobalt and nickel. The FTIR spectra shows shifting of the OH, and C=N functional groups indicating the coordination of the Schiff base with the metal ions. The UV-Vis spectra show ILCT and d-d transactions in the metal complexes. Novel Schiff base (E4CPMA) and three metal complexes were successfully synthesized, characterization using different chemical and physical techniques were carried out. The new synthesized compounds are promising as bioactive materials and it is recommended to test its bioactivity as a further work.

Effect of Sanrego (Lunasia amara Blanco) Stem Extract on Aphrodisiac Activity of Diabetes Mellitus Rats Induced by High-Fat Diet

Abstract : Complications of diabetes are commonly found including the decreased sexual ability for diabetic patients. A plant that is efficacious as an aphrodisiac and is known by people in South Sulawesi is the sanrego plant (Lunacia amara). The aim of this study is to determine the aphrodisiac effect of the ethanol extract of sanrego stems on diabetic rats induced by a high-fat diet. The research method was the post-test method by extracting sanrego stems with ethanol solvent, Wistar rats were fed a high-fat diet for 24 weeks, then HbA1C levels were measured. Rats that experienced insulin resistance continued to the research stage of the aphrodisiac effects of Sanrego which employed four male rats and 16 female rats for animal testing which were divided into four treatment groups. Each group consisted of one male rat and four female rats. Group I was given 1% Na-CMC (negative control), group II was given sildenafil citrate suspension (positive control), group III was given 100 mg/kg BW sanrego stem extract suspension, and group IV was given 150 mg/kg BW sanrego stem extract suspension. It was given orally for seven days and observations were conducted to see how many times the male rats had coitus after 4 hours of treatment. The results of statistical analysis using ANOVA with Duncan's advanced test showed that the administration of ethanol extract of sanrego stem at a dose of 100 mg/kg and 150 mg/kg BW had an aphrodisiac effect compared to Na-CMC as a negative control. Meanwhile, the ethanol extract of sanrego stem at a dose of 100 mg/kg BW and 150 mg /kg BW showed that there was a highly significant difference with sildenafil citrate.

DM EXERCISE AS A STRATEGY TO MAINTAIN BLOOD SUGAR LEVELS IN TYPE II DM PATIENTS

Abstract : Diabetes Mellitus is a chronic disease characterized by an increase in blood sugar levels caused by damage to the pancreas gland as a producer of the hormone insulin, resulting in impaired metabolism of carbohydrates, fats, and proteins. The aim is to analyze the application of diabetes exercise in clients who have diabetes mellitus. The research design is descriptive research, using case study method. Participants consisted of 2 clients who had type II diabetes mellitus, and were taking medication. The results of the implementation of diabetes mellitus exercise showed changes in blood sugar levels in the first client, namely a decrease of 30% and in the second subject, namely 60%. Recommendation: the application of diabetes exercise in DM subjects must be followed by providing information beforehand, being consistent in doing exercises, and motivation to accelerate the healing process and the need for support by the family.

Relationship of Hematological Parameters with State of Heart Failure

Abstract : Anaemia is a common and important comorbidity in patients with chronic heart failure as demonstrated in some studies and is associated with poor outcomes, it is complex and multifactorial. To review some hematological parameters in relation to state of heart failure in term of echocardiographic parameters. A cross sectional hospital based study was to be performed during the period from 15th of February 2019 to 15th of December 2019 enrolling patients admitted to the medical ward, C.C.U. and emergency department at Al-Immamain Al-Kadhimein hospital. For each patient, Demographic information, Clinical information, complete blood count, ECHO study and iron profile in anaemic patients were performed. The mean age of study sample [62.04±11.35] years for anaemic and [62.52±11.90] years for non anaemic patients, female comprises (56.3%) of them, 30% of patients are anaemic, there is no significant statistical association between the anaemic and non anaemic HF patients concerning echocardiographic parameters including LVIDd (P value 0.179), LVIDs (p value 0.605) LA diameter (p value 0.990), septal wall thickness (p value 0.419), as well as aortic root diameter (P value 0.560), also there is no significant statistical relation between EF in anaemic and not anaemic heart failure patients in the current study (P value = 0.644). It couldn’t prove from this study that there is any statistical significant evidence for the effect of anaemia on cardiac chambers geometry or size in patients with heart failure.

Design, Implementation & Evaluation of Communication Skill Module by Using ADDIE Instructional Framework to Enhance Basic Communication Skills among First Year Medical Undergraduates

Abstract : Having an open line of communication between doctors and their patients is crucial to provide excellent care. The importance of include lessons on how to effectively communicate as part of a student's undergraduate medical education cannot be overstated. As part of its mission to improve the quality of medical education in India, MCI/NMC planned to include the AETCOM module to the UG curriculum in 2019. This study's goals were to create, test, and refine the CS module by using ADDIE instructional framework for teaching first-year MBBS students effective communication skills. The current educational interventional study, which used a mixed-method design, was carried out among 100 first-year MBBS students. In order to design, validate, and implement the CS training module based on the AETCOM module, the ADDIE model was used. The CSAS Scale was used to evaluate students' mental preparedness to study CS, while Kirkpatrick's Levels 1 and 2 were used to determine their degree of competence & Visual Anthropac programme analysed qualitative data. Both the positive (4.19 1.03) and negative (2.42 1.24) CSAS component mean scores were statistically significant. The pre- and post-test mean scores for the knowledge component were 4.19 1.03 and 11.64 2.80, respectively (P .05), and cognitive maps were created to establish a causal connection between the key elements of the evaluations' feedback. While students' mean knowledge and skill ratings improved dramatically in the CS category after the first year of MBBS, they still have room for growth in areas like communication and leadership. The students' collective outlook on the importance of communication training improved.

COMPARISON OF TWO DIFFERENT NUTRITION SCREENING TOOLS TO IDENTIFY THE RISK OF HOSPITAL MALNUTRITION AMONG HOSPITALIZED CHILDREN IN INDONESIA

Abstract : Malnutrition among hospitalized children is often misdiagnosed and untreated. This study aimed to compare STRONGkids and PYMS screening tools for determining the risk of malnutrition in hospitalized children. This cross-sectional study was conducted among patients aged 2-to-18 years in tertiary-care hospitals in Indonesia. Demographic data, illnesses, and nutritional assessment based on STRONGkids and PYMS methods were measured within 24-48 hours of admission. The sensitivity, specificity, and positive and negative predictive values of STRONGkids and PYMS for detecting hospital malnutrition were evaluated, with the percentage of weight loss between admission and discharge standing as the gold standard. Out of 48 patients, 54.2% were male with a median age of 11 years. Hematological disease comprised 52.1% of the study’s participants with hospital stays ranging from 3 to 22 days. The prevalence of hospital malnutrition was 25%. The accuracy level of STRONGkids was 70.8% in detecting hospital malnutrition, whereas PYMS was 52%. The sensitivity, specificity, and positive and negative predictive values (PPV and NPV) of STRONGkids for detecting hospital malnutrition were 41.7%, 80.6%, 41.7%, and 80.6%, respectively, whereas PYMS had 83.3% sensitivity, 41.7% specificity, 32.3% PPV, and 88.2% NPV. STRONGkids performed better than PYMS in predicting the probability of hospital malnutrition. Our findings support the use of STRONGkids as a nutritional risk screening tool in hospitalized children in Indonesia.

The Potent of Radical Scavenging, Toxicity, and Apoptosis of Various Fractions of Marine Sponge Axinella aruensis from Kangean Islands

Abstract : A sponge of the family Axinellidae is a marine invertebrate with plenty of medical potential applications. The present study aimed to investigate the antioxidative, toxicity, and apoptotic effects of Axinella aruensis extracts and fractions, particularly from the Indonesian Kangean Islands. The sponge was extracted with methanol, then partitioned with hexane, ethyl acetate, butanol, and water. Each fraction was observed for its free radical scavenging properties to DPPH, toxicity, and apoptosis to HeLa and MCF-7 cell lines. Experimental results show that the ethyl acetate fraction provides the highest activity than the methanolic extract and other fractions. The ethyl acetate fraction gives a strong DPPH radical scavenging with an IC50 value of 27.724 ± 0.029 g/mL and is toxic to Artemia salina with an LC50 value of 24.711 ± 0.478 g/mL. Cytotoxic activity of ethyl acetate fraction against HeLa and MCF-7 cells provide IC50 values of 89.990 ± 2.133 g/mL and 94.143 ± 0.720 g/mL, and induced apoptosis in both cells with IC50 values of 96.659 ± 0.023 g/mL and 93.474 ± 0.013 g/mL. The butanol fraction also showed intense activity, which was relatively comparable to the methanolic extract, although both activities were lower than that of the ethyl acetate fraction. Based on the metabolites fingerprinting analysis results, the ethyl acetate fraction consists of brominated compounds suspected of strongly influencing its anti-cancer properties. The sponge A. aruensis is undergoing further analysis to isolate and characterize the active constituents in each fraction, and thus can be developed for potential anticancer treatment.

Mesenchymal Stem Cell (MSC) Metabolite as a Potential Cell Therapy for Pseudomonas aeruginosa Keratitis

Abstract : Pseudomonas aeruginosa keratitis is the most common eye infection, that develops very rapidly and can lead to endophthalmitis and corneal perforation. Corneal wound healing process is determined by the balance of pro-inflammatory and anti-inflammatory cytokines. Interleukin-1 alpha (IL-1α) and Transforming Growth Factor-β (TGF-β) as “master regulator” cytokines. Mesenchymal stem cells can maintain the equilibrium of these two inflammatory cytokines. The study was to analyze the effect of MSC metabolite to the expression of IL-1α and TGF-β. This study was an animal experimental study. The mice were randomly divided into 3 groups (no treatment, antibiotic and antibiotic-MSC metabolite group). Each group consisted of 6 mice. Keratitis was induced by 5 µl Pseudomonas aeruginosa suspension containing 104 CFU/ml. Isolation of limbus mesenchymal stem cells and cell culture were conducted. The Immunoreactive Score (IRS) and immunohistochemical staining were used to assess IL-1α and TGF- β expression. The mean expression of IL-1α in the antibiotic group with Immunoreactive Score (IRS) was 5.03 ± 3.03, while in the antibiotic + MSC metabolite group it was 4.5 ± 2.51. TGF-β expression in the antibiotic group obtained a mean score of 4.26 ± 2.31, while in the combination group of antibiotics and MSC metabolites the mean score was 2.8 ± 1.30. There was a decrease of the IL-1α and TGF- β expression in the group that was added to the MSC metabolite. The Independent T-Test showed that there was no significant difference between the antibiotic group and the antibiotic + MSC metabolite group p>0.05 (p=0.673 in IL-1α and p = 0.143 in TGF-β expression). Mesenchymal stem cell (MSC) metabolite is a potential cell therapy for corneal wound healing of Pseudomonas aeruginosa keratitis, but further studies are still needed with more sample.
