THE RELATIONSHIP OF PSYCHOLOGICAL STRESS AND SLEEP QUALITY TO FUNCTIONAL DYSPEPSIA IN FIRST YEAR PRECLINIC STUDENTS AND FIRST YEAR STUDENTS CLINIC FACULTY OF MEDICINE SYARIF HIDAYATULLAH STATE ISLAMIC UNIVERSITY JAKARTA

Abstract : Medical students are a population at risk of experiencing various health problems, one of which is functional dyspepsia symptoms. The triggering factors for functional dyspepsia include stress and poor sleep quality. To determine the relationship between psychological stress and sleep quality with the incidence of functional dyspepsia in preclinical first-year students and first-year clinical students at the Faculty of Medicine, Syarif Hidayatullah State Islamic University, Jakarta. The design of this study was cross-sectional with 49 respondents from each sample group selected through the consecutive sampling method from first-year preclinical students and first-year clinical students at the Faculty of Medicine, State Islamic University (FK UIN) Syarif Hidayatullah Jakarta. This research design is cross-sectional with 49 respondents from each sample group selected through the consecutive sampling method from preclinical first-year students and first-year clinical students of FK UIN Syarif Hidayatullah Jakarta. The research instrument used a functional dyspepsia questionnaire based on the Rome IV Criteria, Depression Anxiety Stress Scales (DASS-21), and the Pittsburgh Sleep Quality Index (PSQI). From 49 first-year preclinical students, 42.9% had functional dyspepsia, with 83.3% of them having moderate-to-severe stress levels, and 90.5% having poor sleep quality. Meanwhile, in 49 first year clinical students, 24.5% experienced functional dyspepsia, with 71.4% of them having moderate-to-severe stress levels, and 100% having poor sleep quality. There was a significant relationship between psychological stress and functional dyspepsia in first year preclinical (p=0.001) and clinical (p=0.007) FK UIN Syarif Hidayatullah Jakarta students. However, there was no significant relationship between sleep quality and functional dyspepsia in preclinical (p=0.688) and clinical first-year students (p=0.090) Faculty of Medicine, UIN Syarif Hidayatullah Jakarta. There is a significant relationship between moderate-to-severe psychological stress in students of FK UIN Syarif Hidayatullah Jakarta in the first preclinical year and the first year of the clinic with the incidence of functional dyspepsia. On the other hand, there was no significant relationship between poor sleep quality and the incidence of functional dyspepsia for both medical students in the first preclinical year and in the first clinical year.

Nurses' Work Safety Culture and Nursing Risk Management During the Covid 19 Pandemic In An Effort To Improve Patient Safety (Qualitative Study at Hospital X South Jakarta)

Abstract : Since the government established the Covid 19 Pandemic in April 2020, all health services are required to be able to provide services to Covid 19 patients. Nurses are at the forefront of dealing with Covid 19 patients. During the pandemic, Hospital X has a nursing risk management mitigation strategy to prevent risks. that can occur in the work safety of nurses. However, until the end of 2021 there was an increase in the incidence of nurses exposed to Covid 19 by 8% - 14%. Hospital X must continue to provide quality health services and focus on patient safety during the pandemic. This study aims to explore the pattern of the relationship between nurse work safety and nursing risk management with patient safety in the nursing analysis unit at Hospital X during the Covid-19 pandemic. Descriptive phenomenology qualitative research design. The number of participants involved in in-depth interviews was 17 nurses in the Covid 19 and non-Covid 19 zones. Data analysis used qualitative content analysis techniques with NVivo 12 plus software. The results of the study obtained that the actualization of the work safety culture of nurses during the covid 19 pandemic was running with the support of the leadership, hospital support, compliance with work safety culture, and understanding of nurses. The implementation of nursing risk management also goes with the compliance in work, the involvement of nurses and the prevention of the risk of health problems. The relationship pattern of nurse safety culture has a mutually influencing relationship with nursing risk management and directly affects patient safety. Thus, the hospital management and nursing leadership can continue to provide support that has an impact on the safety of nurses. IPCN and implementing nurses can maintain their role in increasing the knowledge and work compliance of nurses in patient safety efforts.

Physical Functioning of University Students During COVID-19 in Malaysia

Abstract : Students in Malaysia were significantly impacted when a mandatory Movement Control Order (MCO) was issued during the COVID-19 pandemic. Because the COVID-19 pandemic is so severe, it is extremely important to evaluate the physical functioning of students who are socially isolated and do not participate in regular physical activity. In light of the gravity of the situation, the purpose of this concept paper is to shed some light on the state of Malaysian students' physical functioning throughout the COVID-19 pandemic. Understanding how students' bodies functioned during the COVID-19 pandemic in terms of their overall health, risky behaviours, the quality of their sleep, and their eating habits could be essential for the formation of an endemic society and the development of effective measures to deal with catastrophic events.

The Effects of Concurrent Carbogen and Nicotinamide with Chemotherapy on HIF-1α and VEGF Serum in DLBCL Patients

Abstract : To investigate the effect of additional Carbogen Nicotinamide (CON) to standard R-CHOP chemotherapy on HIF-1α and VEGF serum in DLBCL patients with tumor hypoxia. This is an interim analysis of an open-label clinical trial conducted at Dr. Kariadi Hospital Semarang. Newly diagnosed DLBCL patients with HIF-1α overexpression ≥10% were included and randomized into study group (R-CHOP plus nicotinamide and carbogen 10 liter/min) and control group (R-CHOP). From July-December 2021, 20 patients were enrolled and randomized. No significant differences in baseline clinical and laboratory parameters between both groups. Significant reductions were found in both HIF-1α (p=0.031) and VEGF (p=0.022) serum levels post-chemotherapy in the study group, however, no significant difference was found in the control group. CON (p=0.035; OR 9.3, 95%CI=1.2-72.9) and bulky tumor at presentation (p=0.012; OR 15.7, 95%CI=1.7-141.4) were significantly associated with the reduction of VEGF serum. The addition of CON in DLBCL patients could reduce the HIF-1α and VEGF serum levels significantly. Further studies are needed to confirm the positive effects of this intervention.

IN SILICO STUDY: MOLECULAR DOCKING TARGETING KRAS RECEPTOR IN LUNG CANCER

Abstract : Indonesia is a mega-biodiversity country with a high number of herbal plants, in particular Murraya koenigii and Nigella sativa. Their extract reported having activities as anti-cancer, including lung cancer. It is one type of cancer which has a high mortality rate. One of the important receptors for lung cancer is Kirsten rat sarcoma virus (KRAS) which grows in lung adenocarcinoma, a subtype of Non-small Cell Lung cancer (NSCLS). Recent treatment such as sotorasib could cause side effects and serious adverse effects, thus an alternative treatment is needed. The currently developed method for skrining new compounds targeted KRAS receptors is molecular docking. This study aims to find candidates for several ligands that have interactions with the KRAS receptor in lung cancer to have better activity than sotorasib. We carried out an in silico study by docking candidate ligands with the KRAS receptor using the MOE 2015 V.10 application. 5 out of 10 candidate compounds have interactions through hydrogen bonding with the KRAS receptor. Dithymoquinone is the compound with the lowest Gibbs free energy, which is -12.3107 joules/kg.mol, and has the strongest bond. Dithymoquione derived from the Nigella sativa (Black cumin) corresponds to sotorasib for ligand-interaction when docked with the KRAS G12C receptor. This finding could be a potential new drug for lung cancer. However, further studies, such as in vitro, in vivo and clinical trials need to be conducted to confirm the activities, safety and efficacy of the new drug.

Common clinical patterns of fixed drug eruptions among Iraqi patients. A multicenter study.

Abstract : The purpose of this study was to assess the common clinical presentations of fixed ‎drug eruption (FDE) among Iraqi patients.‎ This is a multicenter cross-sectional descriptive study. One hundred and sixty patients who had FDE were seen in the Departments of Dermatology and Venereology at Al-‎Saddir Teaching Hospital and Ibn-Sena Teaching Hospital, Iraq, during the period from Oct. 2017 to Oct. 2018. A full history and clinical examinations were done for every patient. An oral provocation test was done for some patients. A skin biopsy was done for some patients to confirm the diagnosis. The age of the patients was 37.2 ± 16 years. The male-to-female ratio was 1.2:1.‎ Most of our patients presented during the first week of rash appearance as they ‎constituted 77.5% of patients. The shortest time of presentation after taking the causative medications was 30 ‎minutes, whereas the longest duration from taking the insult medication was within ‎two weeks. ‎ Multiple lesions were encountered in 76% of the patients and the upper extremities ‎were the most frequently affected area in 26% of the patients. The most common morphological presentation in a patient with FDE was patches ‎‎49% and erosive type 17%. Itching was the most ‎common symptom suffered by 42% of patients. The most common culprit agent of FDE was co-trimoxazole 47.5%. FDE is a common cutaneous disorder seen in dermatological outpatient clinics. Recurrent multiple patchy and erosive lesions are ‎the commonest clinical presentation. The upper extremities are the most anatomical sites affected by FDE. Pruritus is the most commonest associated symptom.‎

Life-Quality Assessment of Bladder-Cancer Patients via SF-36 Questionnaire Undergone the TURBT, Chemoradiotherapy and Radical Cystectomy at H. Adam Malik General Hospital Medan

Abstract : Untreated bladder cancer has a high mortality rate. In bladder cancer patients, urinary symptoms and sexual function are often assessed. Use of the SF-36 Questionnaire to assess disease-related quality of life (QoL). Current use in bladder cancer is restricted. The SF-36 was used to assess QoL in patients with bladder cancer at H. Adam Malik General Hospital Medan. This study was designed to assess the QoL bladder cancer patients undergoing these therapeutic protocols via the SF-36 questionnaire. The study included all patients with bladder cancer treated with TURBT, chemoradiotherapy, or radical cystectomy at H. Adam Malik General Hospital Medan's Department of Surgery. From January to December 2020, we recruited. The SF-36 questionnaire was used to assess QoL. A total of 36 patients were enrolled in this study with male predominance (66.67%) and the mean age was 55.64 ± 14.1 years. Muscle invasive was found to be the most diagnosed stage of bladder cancer (66.67%), and majority of the patients underwent TURBT (55.56%). All therapeutic protocols were mainly carried out in muscle invasive stage, while those undergoing the therapeutic protocols in H. Adam Malik General Hospital had a lower value (<50) than the normative value (50) which indicated a poor QoL. The QoL of bladder cancer patients undergoing TURBT, chemoradiotherapy and radical cystectomy were poor.

Prevalence of inferior alveolar nerve block failure among undergraduate dental students: susceptible reasons and how to avoid them?

Abstract : Inferior alveolar nerve block (IANB) is the main technique for anaesthetization in the mandible. Profound inferior alveolar nerve block (IANB) aesthesia remains difficult to achieve at all cases. Multiple causes of failure or incomplete anesthesia were reported. To assess IANB failure rate among dental students and instructors, the susceptible causes of failure, and how it is overcome. In addition to investigate the awareness of alternative techniques of IANB and the rate of IANB-related complications. A questionnaire containing twenty questions was distributed for 300 of dental students and interns and also for 100 of instructors at the vision colleges /Jeddah -Riyadh/Saudi Arabia. It included demographic questions, questions on IANB failure rate and causes, how the failure is usually overcome, awareness of different alternative anaesthetic techniques and the most common encountered complications. 356 participants were involved in the study. Most of surveyed sample (91.3%) had experienced IANB failure. The most commonly reported causes of failure were unclear landmarks (46.1%) and anatomical variations (38.5%). The most common alternative technique used was intraligamentary injection (49.4%). The most common reported complications according to our respondents were facial paralysis (17.1%) and trismus (16.3%). More training of students on IANB injection is needed. In addition alternatives to IANB, such as the Gow-Gates and Akinosi techniques should be activated and emphasized, theoretically and clinically, in the anesthesia courses of the dental curriculum.

THERAPY OF BIOLOGICAL AGENTS IN PSORIASIS VULGARIS THAT FAILED METOTREXATE TREATMENT: A CASE REPORT

Abstract : An inflammatory skin condition that can affect people of all ages is psoriasis. Each population has a different psoriasis prevalence level, ranging from 0.1 to 11.8%. Depending on the severity of the condition, numerous treatment options are available, including topical therapy, phototherapy, systemic therapy, and biological medicines for psoriasis. We reported that a 35-year-old man complained of having thickened and scaly skin across most of his body four months ago. Since seven years ago, the patient has frequently complained of the same illness and has received oral methotrexate and phototherapy. An examination by a dermatologist revealed well-defined erythematous plaques, erythematous macules, and thick gray scales in the generalized area. Blood sugar, liver function, kidney function, and the results of routine blood tests were all within normal ranges. The histopathological analysis showed hypogranulosis, moderate spongiosis, psoriasiform hyperplasia, and dilated papillary dermal blood vessels carrying erythrocytes. This patient received five cycles of Secukinumab 150 mg subcutaneous injection therapy following the diagnosis of psoriasis Vulgaris that had been made. Each cycle of therapy resulted in a progressive clinical improvement, and by the fourth cycle, 50 days after therapy, the hyperpigmented patch lesions had diminished, and the scales were becoming thinner. In conclusion, with satisfactory results, the biological drug Secukinumab can be used as a second-line treatment for psoriasis Vulgaris, notably when methotrexate therapy has ineffective.

Comparative structural organization of the glands of the mucous membrane of the inferior and posterior walls of the normal human frontal sinus

Abstract : The present paper was aimed at the study of the peculiarities of the mucociliary system of the mucous membrane normal human frontal sinus (FS). Cadaveric material obtained from people of both sexes aged 26 to 75 years who died of causes not related to pathology of the paranasal sinuses has been studied in accordance with international guidelines for biological research. After obtaining the mucous membranes of the FS, their fragments were fixed in phosphate buffered 2.5% glutaraldehyde solution and embedded in the Epon-812 epoxy resin. The obtained preparations were studied on the “Konus” light microscope, equipped with the Sigeta DCM-900 9.0 MP digital microphoto attachment and the Вiorex 3 (serial number 5604) software, adapted for the above studies. The resulting morphometric digital data were subject to statistical processing on a personal computer using the Statistica 13 and Microsoft Excel 2010 software packages. The investigated glands are complex alveolar-tubular glands, which consist of a system of excretory ducts with the acini. The findings of our study showed a certain difference between the structure of the acini of the glands on the inferior wall, where they were divided into 2 types. The first type is characterized by the formation of the acini by the cuboidal cells, and the second type by the pyramidal cells. By the secretory products, the glands of the inferior wall are defined as proteinaceous and mucous, and the glands of the posterior wall are mixed. The morphometry showed that the values of the average thickness (µm) of the submucous layer of the posterior wall (PW) on both sides were by 74% lower compared to the parameter for the inferior wall (IW). No significant difference in the outer diameter (µm) of the acini of the glands of the specified walls on both sides was found.
