Abstract : Obsessive-compulsive disorder (OCD) is a chronic psychiatric illness characterised by unwanted, repeated thoughts (obsessions) and/or the repetition of rituals or activities(compulsions). It has a global lifetime prevalence rate of 1.5 percent for women and 1.0 percent for men. Aim: To assess the biological and psychosocial factors of a person with OCD and to provide appropriate psychotherapy to the client. A single case study design was carried out with the case of obsessive-compulsive disorder. Post intervention showed a significant reduction of OCD and depressive symptoms. Exposure response prevention (ERP) has been shown to be effective in treating OCD in this case, as evidenced by the post-intervention reduction in OCD symptoms. Implication: ERP is effective guideline for the practitioners to manage OCD symptoms, which is evident from the current case study.

EFFECTIVENESS OF COMBINATION OF AIR CUSSION AND 30 DEGREE LATERAL POSITION TO PREVENT PRESSURE INJURY IN BED RIDDEN STROKE PATIENTS
Journal ID : AMJ-01-08-2022-10343; Author : Fitri Anita, Nur Chayati,
Abstract : One of the effects of stroke is the occurrence of hemiparesis or paralysis which makes the patient bed rest for a long time, causing the risk of pressure injury. Several pressure injury prevention interventions have been mentioned in previous studies, but with varying methods and results. The purpose of this study was to determine the effectiveness of using an air cushion and a 300 lateral position in reducing the risk of pressure injury in stroke patients. This study uses a quasi-experimental method with pre and post-test with a control group design. The minimum sample size required is 17 patients for each group (intervention and control). The group was given a combination of air cushion and right and left lateral position 300, while the control group was given daily intervention (standard) ward maintenance. The duration of the intervention was 3 days. Assessment of the risk of pressure injury using the Braden scale, and monitoring of changes in position using the observation sheet. Pressure ulcer risk assessment is carried out every morning. The data were statistically tested using the Wilcoxon test and the Mann-Whitney-U test. Data analysis showed that the combination of an air cushion and a 30º position effectively reduced the risk of injury p-value of 0.035 (<0.05). While the control group obtained a p-value of 0.480 (> 0.05), which means that the daily intervention by the control group nurse proved ineffective in reducing the risk of pressure injury in stroke patients. The Mann-Whitney test obtained a p-value of 0.009, meaning that the combination of an air cushion and a 30º lateral position in stroke patients with bed rest was effective in reducing the risk of pressure injury compared to the control group. This study proves that the prevention of pressure injury by giving a combination of air cushions and a 30° lateral position is effective for at least 3 days.

Padiatric BCG Vaccinee and IgE Responses
Journal ID : AMJ-01-08-2022-10340; Author : TIBA AHMED KAREEM, IBRAHIM M S SHNAWA, HANEEN A. H. ALABDILY,
Abstract : Fifteen from each of the study groups selected on the bases of simple random sampling were blood collected and sera saved. Total IgE assay was done to each serum in duplicate for each of the individuals forming these test groups using the ELISA IgE Kit. Minimum IgE concentrations for the BCG vaccine groups were found to be lower than that of the nonvaccine control group. This may be attributed to the presence of a tolerogenic epitope ensemble within the BCG antigenic structure. BCG vaccine groups were showing higher IgE concentration means than the control groups. A finding which can be attributed to the presence of atopic allergenic epitope in BCG structure The non-scar bearing BCG vaccinee was showing higher IgE concentration means than those of scar bearing BCG vaccinee. Though these higher mean values may be statistically non-significant. BCG scar fading may be due to the waning of responsible cytokine responses in scar formation as the time post-vaccination prolonged. BCG vaccination in childhood initiates variable degrees of IgE responses among vaccine.

Journal ID : AMJ-31-07-2022-10337; Author : Anita Indah Sari, Khairuddin Djawad, Airin R, Nurdin Mappewali, Idrianti Idrus,
Abstract : Basal cell carcinoma (BCC) is the most common type of skin cancer and frequently appears in the centre of the face, particularly the nose. Long-term exposure to ultraviolet radiation is a risk factor for BCC. Biopsy findings routinely confirm the clinical diagnosis. Complete surgical resection is most commonly used in the treatment of facial skin cancer, and the methods used for defect closure are primary closure, local flaps and skin grafts. This article reports a 73-year-old man who complained of a blackish spot on the side of the nose and infra-orbitalis dextra that occurred about two years ago. The complaint was accompanied by itching and bleeding when scratched. The patient was diagnosed with basal cell carcinoma from the history, physical examination, dermoscopy examination, and histopathology. The surgery is carried out in two stages: first, Modified MOHS surgery and secondly, defect closure is performed using the Paramedian Forehead Flap, Rotation Flap and Skin Graft. On the 28th day of follow-up, the surgical wound had dried up completely. The combination surgical technique of paramedian forehead flap, rotation flap, and skin graft can be a treatment option in large facial basal cell carcinoma to achieve an excellent cosmetic and functional outcome.

Detection of Gastrointestinal Lesions Causing Iron Deficiency Anemia by Upper With or Without Lower GI Endoscopy in the Egyptian Society
Journal ID : AMJ-31-07-2022-10335; Author : mohamed sayed, mohamed mahmoud Ali, Taha Mohammed Masri, Mahmoud Wahba, Mohamed Badr Hassan,
Abstract : Most gastrointestinal lesions cause iron-deficiency anemia. Endoscopy of the upper and lower digestive tracts is indicated for IDA patients because most sources of gastrointestinal hemorrhage can be found. Clinical history can predict positive GI endoscopic findings, but its sensitivity in detecting specific lesions, including GI neoplasia, is limited. This study included 1064 IDA patients who had upper and lower GI endoscopies to determine the efficacy of upper and lower endoscopy in diagnosing gastrointestinal causes of iron deficiency anemia in Egyptian individuals. According to our findings, the most common cause of Fe deficiency anemia in upper gastrointestinal lesions (39.8 %) was peptic ulcer, whether gastric or duodenal, followed by angiodysplasia (11.85 %). Colorectal cancer (31.4 percent) was the most common cause of Fe deficiency anemia in lower gastrointestinal lesions discovered by colonoscopy, followed by lower gastrointestinal angiodysplasia (2.7 %). There was a high prevalence of neoplasm in IDA patients. 303 of 700 patients with iron deficiency anemia developed gastrointestinal neoplasm, with 82 instances pathologically proving to be upper gastrointestinal adenocarcinoma and 221 cases being colorectal cancer. In 18 IDA patients, combined lesions were seen. Upper and lower GI endoscopies are beneficial in diagnosing Iron deficiency anemia, says this study. In elderly patients with iron deficiency anemia, gastrointestinal cancers are prevalent. Peptic ulcers are the most prevalent upper GI lesion in Egyptians.
