The Effectiveness of Dequalinium Chloride Compared to Metronidazole as a Therapy for Bacterial Vaginosis

Abstract : Centers for Disease Control and Prevention (CDC) recommends antibiotics metronidazole and clindamycin as the first-line therapy for bacterial vaginosis. It correlates with the number of antibiotic resistances. Therefore, an alternative to antibiotic, such as antiseptic, is sought to deal with these problems. Dequalinium chloride (DQC) is one of the antiseptics that has been widely studied as a therapy for bacterial vaginosis. The broad spectrum activity of DQC increases its effectiveness against bacterial vaginosis. This double-blind randomized controlled trial was carried out at Dr. Cipto Mangunkusumo General Hospital from November 2020 – October 2021. The effectiveness of Dequalinium chloride was compared with Metronidazole as control (17 subjects for each group). The randomization was performed using block randomization. The outcome that was reported in this study was the cure rate as well as the side effects experienced by the users. The cure rate of subjects in treatment (DQC treatment) group was 94.12%; as compared to 100% in control groups. The difference in cure rate was not statistically significant (p = 0.50). No subjects reported any side effects in both groups. Dequalinium chloride is a safe and effective alternative to Metronidazole in treating bacterial vaginosis.

Breast Mass Among Young and Adolescent Attending Al-Elwiya Teaching Hospital

Abstract : The purpose of this study was to evaluate the incidence and clinical manifestations of benign breast lumps in adolescents admitted to a Al-Elwiya Teaching Hospital. All adolescents who visited the outpatient department (OPD) with breast lump complaints were enrolled in the study. Patients were a one hundred cases with breast lump with age of 25 years or less. All patients had assessment in the form of short history, clinical examination, ultrasound examination and fine needle aspiration cytology (FNA). As fibroadenoma found in 10% of all women according to the Autopsy studies. Regarding body mass index, no statistical difference was found. According to the occupation and educational level, no difference in the diagnosis was found. Marital status had no difference on the diagnosis of breast mass. Nulliparity (together with gravidity, parity and miscarriage) were associated with high rate of fibroadenoma. The side of the breast mass, past surgical history, past medical history, smoking all were not different in regard to the diagnosis of breast mass. Although, both family history and 1st and 2nd degree relatives with benign breast disease were not significant different in regard to the diagnosis of breast mass; all cases of positive family history of fibroadenoma had the similar diagnosis in both 1st and 2nd degree relatives. Most common cause of benign breast mass is fibroadenoma. Fibroadenoma was more common in age group ranged from 15-21 years, cases of nulliparity, positive family history and hormonal treatment.

Changes in sitting height ratio of Yogyakarta children aged 7-15 years

Abstract : Poor health status, unhealthy behavior, nutritional status, and lifestyle can cause disproportionate growth, and body proportions can be seen from the sitting height ratio. This study aimed to evaluate the changes of sitting height ratio of Yogyakarta boys and girls aged 7-15 years. A cross sectional data was performed on 492 Javanese children consisting of 237 boys and 255 girls, aged 6-12 years. The body height and sitting height of each subject was measured in order to calculate the ratio between sitting height and height. Anthropometric measurements were measured to all subjects based on the International Standard for Anthropometric Assessment procedures. Data were analyzed using ANOVA, Chi-Square tests, and simple linear regression analysis with a significance of p<0.05. There were significant differences between the average sitting height and sitting height ratio between boys and girls. Average height and sitting height of boys was 140.18 cm and 71.93 cm, while girls were 142.58 cm and 74.25 cm, respectively. There was a positive correlation (p<0.05) between sitting height, height, and sitting height ratio with age in both sexes. Among children the older the age, the greater the sitting height ratio. Sitting height ratio can be used to assist in the diagnosis and follow-up of children with growth problems.

¬Comparison of sperm parameters improvement and complications between retroperitoneal and microsurgical varicocelectomy

Abstract : Varicocele is one of the causes of male infertility that can be corrected surgically. There are several varicocelectomy surgical techniques, including retroperitoneal technique and microsurgical technique. This study aims to compare the sperm parameters improvement and complications between retroperitoneal and microsurgical techniques. This study is a prospective cohort study. Data were taken from the medical records of patients who underwent varicocelectomy surgery from January 2016 to February 2019. A total of 100 patients, with grade 2-3 varicocele, divided evenly into retroperitoneal technique group, and microsurgical technique group. The same surgeon performed the operations. The sperm parameters before and after surgery, postoperative hydrocele, and the degree of postoperative pain were evaluated. There was a significant improvement in postoperative sperm parameters in both techniques. Whereas the results of postoperative sperm parameters between both techniques didn’t show a significant difference. No postoperative hydrocele was found in both techniques. A mild degree of postoperative pain was found in all study subjects in the microsurgical group. Whereas in the retroperitoneal group, mild postoperative pain was found in 11 subjects, and moderate postoperative pain was found in 39 subjects. Varicocelectomy provides a significant improvement in the postoperative sperm parameters. Both techniques provide equal significant improvement in sperm parameters with fewer hydrocele complication events. The microsurgical technique provides a better degree of postoperative pain than retroperitoneal techniques.

Effect of metformin on gamma radiation -Induced DNA oxidation damage in Human Peripheral Blood Lymphocytes in vitro.

Abstract : The aim of the study was to assess the effect of metformin (an anti-hyperglycemic drug in type 2 diabetes (T2D) patient) on DNA oxidative damage in vitro in human peripheral blood lymphocyte from 10 donors; evaluation of the oxidative damage induced by 1 and 2 Gy gamma rays; and assessed the role of metformin in protecting lymphocytes against the oxidative damage induced by gamma rays. The 8-Hydroxydeoxyguanosine (8-OHdG) ELISA Kit was used to detect the DNA oxidative damage in blood lymphocytes. Metformin (10 and 50µM) in vitro increased the rate of (8-OHdG) significantly (P ≤ 0.001) than control samples value. Irradiated blood lymphocyte with 1 Gy gamma rays elevated the DNA oxidative damage in rate (95%) and significantly (P ≤ 0.001) than control values, and the rise of irradiation into 2 Gy, caused increase the DNA oxidative damage (8-OHdG) 2.3 folds and significantly (P ≤ 0.001) than 1 Gy irradiated. Blood treated with metformin prior irradiation enhance the reduction of (8-OHdG) specially in blood irradiated by 2 Gy gamma ray, the reduction significantly (P ≤ 0.05 and P ≤ 0.001) respectively for (10 and 50 µM) metformin treated compared with irradiated group only.

Foundational Competencies of PHNs in Dealing with Disaster Recovery Phase: An Integrative Review

Abstract : Disasters have continued worldwide, and these events constitute an essential concern for nurses in pursuing a strategic map to address this issue. As a discipline that plays a fundamental role in delivering disaster care, PHNs will be part of the discipline and are expected to participate actively in disaster management programs. This review paper investigates the foundational competencies of PHNs in dealing with the disaster recovery phase. An integrative review approach is achieved with various sources of information from an intensive search of the related literature by multiple methods. In developing the foundational competencies of PHNs in this study, nurses' knowledge, skills, and competencies in the disaster recovery stage launched by the organization's review papers and research papers will be discussed. Literature was searched by using a keyword combination as follows: "disaster mitigation" or "disaster preparedness" or "disaster-prone population," and "disaster recovery phase" and "the role of nurses" or "community disaster nursing. The main focus on PHNs competencies at this stage is assessing public health impacts and health promotion for the community. To participate actively during this period, they must possess knowledge and expertise in disaster management to work effectively in the event of a disaster. Moreover, intensive and sustained health promotion must be improved to raise awareness of potential risks to their quality of life following a disaster or catastrophe.

Prevalence of temporomandibular joint problems in various occupations in western region Saudi Arabia during the COVID-19 pandemic

Abstract : The study aimed to determine the severity and prevalence of temporomandibular disorders (TMDs) in people from different professions in Saudi Arabia's western region to identify which one subjected to more stressful situation. The present cross sectional study was performed using a standard questionnaire-based study with Fonseca Anamnestic Index. The study was performed on adult participants from western area of Saudia Arabia who agreed to participate in the questionnaire between August 2020 and October 2020. The study included 1280 participants. The study sample were divided into six groups according to their professions (health care providers, business, college student, education, engineered and un-employed). Data were statistically analyzed for significance and correlations. The sample included 999 males and 281 females ranging in age from 18 to 65. Mild TMDs symptoms are reported in nearly half the participants. There was non-significant difference between different professions of the participants. The new pandemic COVID-19 may affect the health status among population of the society leading to mild symptoms related to temporomandibular joint regardless to their professions.

NUCLEAR MATRIX PROTEIN (NMP) 22 PERFORMANCE IN THE DIAGNOSIS BLADDER CANCER: A SYSTEMATIC REVIEW AND META-ANALYSIS

Abstract : One of the most common cancers occurring worldwide is Bladder Cancer. In 2018 there were 549393 cases of bladder cancer along with 199922 deaths. Early detection is essential to prevent the disease progression and improve the patient’s survival and quality of life. In this study, we assessed the performance of Nuclear Matrix Protein 22 (NMP22) for the early detection of bladder cancer. We searched kinds of literature using PubMed, MEDLINE, and Cochrane Library. The population was Bladder cancer patients undergoing the NMP22 test. The diagnostic accuracy of the NMP22 was evaluated through sensitivity, specificity, Positive Likelihood Ratio (PLR), Negative Likelihood Ratio (NLR), Diagnostic Odds Ratio (DOR), and Area Under Curve (AUC). Heterogeneity was analyzed using subgroup analyses. Egger test was used to detect publication bias. Stata 14.0 and Review Manager were used to analyze the data. The results showed sensitivity was 0.64 (95% CI = 0.57 to 0.70), specificity was 0.84 (95% CI = 0.79 to 0.88), PLR was 4.1 (95% CI = 3.1 to 5.4), NLR was 0.43 (95% CI = 0.36 to 0.50), DOR was 10 (95% CI = 7 to 14), and AUC was 0.81 (0.77 – 0.84). NMP22 had high sensitivity and specificity in ASIA compared to other regions. Egger’s test showed that no publication bias existed (P = 0.36). NMP22 test could become an early detection method for bladder cancer because of its high specificity. The NMP22 test may be appropriate for detecting bladder cancer, but it cannot replace cystoscopy.

Intra-Aortic Balloon Pump Catheter Tip Localization by Measuring the Perfusion Index Using a Designed Sensing System and Tracking the Blood Flowing Through the Kidney and The Upper Extremities

Abstract : The intra-aortic balloon pump is the most widely used mechanical assist device for hemodynamic support in high-risk patients undergoing heart surgery (IABP). The purpose of this research is to determine if the catheter is in the best possible position, as poor positioning might result in adverse side effects such a blockage of the subclavian or renal arteries. This study comprised ten individuals with severe coronary artery disease who received coronary artery bypass grafting. These patients could not be weaned off cardiopulmonary bypass and needed IABP. The designed system of sensors was used to measure the perfusion index and doppler for monitoring blood flow through the hand and kidney during the insertion of the IABP catheter into the femoral artery. All the patients underwent the surgery satisfactorily. IABP assistance lasted an average of hours. The use of this sensing device was not associated with any in-hospital mortality or problems. This method is straightforward, dependable, and aids in the reduction of unwanted balloon catheterization side effects.

Serum level of periostin in patients with Type 2 Diabetes Mellitus in Bsrah, Iraq

Abstract : Diabetes mellitus is a group of metabolic disorders characterized by the presence of hyperglycemia in the absence of treatment. The heterogeneous aetio-pathology includes a defect in insulin secretion, action, or both. Periostin is a matricellular protein that is a member of the fasciclin (fasc) family and has structural similarities with the transforming growth factor—β inducible protein and insect axon guidance fasciclin1. The objective of this study was to comparison of the serum level of periostin in patients with T2DM and control to assess if there is an association of periostin with diabetic control in diabetic patients and to study the effect of age and duration of disease on the serum level of periostin. In the present study, included 175 participants categorized into two main categories; 89 patients with type 2 diabetes mellitus as cases, and 86 healthy individuals matched for both age and sex with cases. Body weight, height, and waist circumference were measured and body mass index was calculated for each participant. Overnight fasting blood samples (5-7ml) were collected. A part of (2ml) from each drawn sample was dispensed in a tube containing K3EDTA to measure glycated hemoglobin. The rest of the blood samples were placed in a serum separator tube, they were centrifuged to get the serum to measure fasting blood glucose, lipid profile, and periostin.
