Evaluation of the antioxidant and antimicrobial potential of Vaishvanara Churna: A polyherbal formulation

Abstract : This experimental work was focused to study the anti-oxidant and anti-microbial potential of Vaishvanara Churna– a polyherbal traditional ayurvedic formulation generally recommended for rheumatism (amavata), flatulence with gurgling sound (adhmana), abdominal lumps (gulma), duodenal ulcer (parinamasula), heart disease (hardya roga), etc. In order to establish the scientific pieces of evidence, the formulation was evaluated for anti-microbial potential using some gram-positive (S. aureus, Clostridium sporogenes, Bacillus subtilus.), gram-negative (Escherichia coli, Klebsiella pneumoniae, Shigella boydii, Pseudomonas aeruginosa) bacterial and fungal strains such as Candida albicans, Aspergillus brasiliensis. Additionally, the lyophilized hydroalcoholic Extract (LHE) was evaluated for antioxidant potential using 2, 7-dichlorofluorescein diacetate (DCF-DA) by employing macrophage J774A.1 cell lines and 2, 2-diphenyl-1-picryl-hydrazyl-hydrate (DPPH) method. Several fractions of the polyherbal formulation Vaishvanara Churna (5-100 mg/ml) was found to be significant antimicrobial potential against selected bacterial and fungal strains except Aspergillus brasiliensis. The antioxidant efficacy of the LHE was also found to be significant in both of the selected methods. Conclusively, this research study established the scientific evidence for the antibacterial, antifungal and antioxidant potential of Vaishvanara Churna formulation supporting its traditional therapeutic use.

Health-Related Quality of Life and Oxidative stress markers in Homozygous and Heterozygous Sickle cell Anemia in Central India

Abstract : Analysis of oxidative stress markers and treatment of sickle cell anemia are important to protect the people from getting affected and minimizing the level of stress to maintain a healthy lifestyle. Apart from various complications most families with sickle cell child have to bear with social problems and financial crisis. Broad spectrum multidimensional concept is Health Related Quality of life (HRQoL). A Comparative cross-sectional study was carried in 240 Diagnosed sickle cell anaemia cases (120 Homozygous SS and 120 Heterozygous AS) and 120 controls. Oxidative stress markers Malondialdehyde (MDA), SOD Superoxide dismutase, glutathione peroxidase and catalase were analysed in all subjects and data was analysed using descriptive and inferential statistics using ANOVA and statistical significance was decided by p value. Mean MDA in SS cases is 4.15 m mol/L, AS is 2.44 m mol/L, and control is 1.81 m mol/L, showing a variation across the three groups (p<.05) statistically significant. Mean Superoxide Dismutase of SS is 318.81 U/ml, AS is 252.39 U/ml, and control is 199.28 U/ml, showing a significant difference in three groups. In addition to this, the mean Catalase of SS is 34.68 nmol/min/ml, AS is 34.58 nmol/min/ml, and control is 39.69 nmol/min/ml, showing a significant difference in Catalase between the three groups (p<.05). Oxidative stress is more in homozygous than heterozygous sickle cell anemia. The HRQoL of patients of sickle cell anaemia was noted to be significantly lower as compared with the healthy controls.

COMPARISSON OF PARAMETERS IN POSTOPERATIVE RADIOLOGICAL EVALUATION BETWEEN DORSAL BRIDGE PLATE AND VARIABLE ANGLE PLATE IN INTRAARTICULAR DISTAL RADIUS FRACTURE

Abstract : In this study, we want to evaluate the outcome of each technique based on radiological alignment in order to give consideration of its treatment. This study use purposive sampling with cross sectional design study to gather data in radiological examination patients post intraarticular distal radius fracture that fixated using dorsal bridge plate or variable angle plate based on surgeon preference, then we measure for its radial height, radial inclination, articular step-off, volar tilt, ulnar variance in preoperative and post operative. There is a significant decrease of radial height and ulnar variance in Dorsal Bridge Plate (DBP) and significant decrease of radial height and volar tilt in Variable Angle Plate (VAP) (p<0.005). The use of variable angle plate compare to dorsal bridge plate has the same result based on postoperative radiological measurement finding for each parameter. Radiographic results may affect post operative function. The majority of the patients had a good radiographic score which correlated positively with the objective functional score. Dorsal bridge plate can be used as an alternative in managing intraarticular distal radius fracture due to its radiograph and functional outcome that may be considered as an appropriate treatment.

Improving early detection of tuberculosis cases among household contacts through home visits by community health workers

Abstract : One of the problems in controlling tuberculosis is the delay in diagnosis, this is at risk of increasing the possibility of transmission in the household and community. Early detection through home visits is an effective method for detecting new cases of tuberculosis by empowering Community Health Workers (CHWs). This study aims to analyze the relationship between home visits with detection of new cases tuberculosis among household contacts by CHWs. The design of this study was cross-sectional descriptive survey, using a quantitative approach, carried out from July to September 2021 at 26 Public Health Centers in Samarinda. The research sample were 304 CHWs using purposive sampling. Bivariate analysis test using chi square showed a significant relationship between home visits and detection of new cases of tuberculosis in household contacts (p-value = 0.000) and OR = 3.685 (95% CI). Early detection by CHWs through home visits is very effective in finding new cases tuberculosis of household contacts. It is necessary to increase CHWs participation, improve coordination among CHWs and Health Center Personnel, certified training and adequate facilities.

Awareness of thalassemia intermedia caregivers about iron overload in Baghdad

Abstract : Thalassemia is a hereditary haemoglobinopathy, chronic disorder requiring blood transfusion for lifelong that cause financial and emotional burden on the family and society. This study was done to assess level of awareness in the caregivers of thalassemic patients to ameliorate their experience in caring their child. A cross sectional descriptive study was conducted in AL-Karama and IbnAL- Baladi thalassemia centers in Baghdad from november2020-june 2021 in which 220 caregivers of thalassemia intermedia patients were interviewed to assess their level of awareness, the practice they exert regarding to blood transfusion, iron over load and its treatment. 52% of the caregivers had good awareness, 46% had fair awareness and2%had poor awareness level. 11.8% thinks blood transfusion must be given from time to time, 28.4% thinks iron overload means increase level of iron in the body, Liver cirrhosis and heart failure was the most important complication of iron overload if not treated as 88% of care givers declare, chelating agents was one of the treatments of iron overload for 97.3%, Oral or subcutaneous chelating agents we rementioned by 99.1%as types of treatment, 98.2% know that serum ferritin and type of thalassemia was the most indicators for patients to take chelating agents, About 11.8% of patients stated that chelating agents prescribed according to patients will and his parents, burning sensation in site of injection and GIT upset was one of side effects of chelating agents in the opinion of 67.3%, 84.5% consult a doctor before stop treatment when side effect appears, 50% of studied sample stated that they are going to change treatment from one type to another if liver enzymes elevated,77.3%knows that the treatment is lifelong. Age and educational level were significant predictors in relation to level of awareness of the caregivers; Majority of caregivers had good level of awareness and enough knowledge about thalassemia, Management should include comprehensive education and increase level of awareness between caregivers and patients them selves and enhancement of management strategies existed. To assess the level of awareness among caregivers with thalassemia intermedia. To assess level of awareness about thalassemia intermedia complication (iron overload) and the types of it’s treatment, it’s side effect. To findout associations between socio-demographic variables and awareness inthalassemic intermedia patients.

An investigation of the impact of wellbeing programs in addressing workers living with cancer workplace issues: An Integrated Literature Review

Abstract : There are a rising number of workers living with Cancer globally. The cancer burden projections indicate a potential gradual rise in the proportion of employees living with Cancer globally. The increase in employees' number proportion necessitates the need for the employees' issues identification and mitigation. The review develops a critical examination of some of the unique workplace issues faced by the employees' category and how the adoption of the organisational wellness programs improves the employees' wellbeing. The review collects secondary data from CINAHL, MEDLINE, Scopus, and Google Scholar, respectively. Key search words and phrases including Wellbeing programs AND resolving OR addressing AND workers living with cancer AND workplace issues were used. The review used the GRADE model to assess the review article quality and select the most applicable ones. An inclusion and exclusion criteria were used to determine the studies to include in the review analysis. The findings indicate that there are unique workplace issues facing workers living with Cancer. The key issues include the fear of victimisation, anxiety, stress, and health complications. The issues are more pronounced in this workers category than in other clusters. In addressing the issues, the review establishes that organisations used different wellness programs, including support groups, exercise, and medical screening and diagnosis programs. The review concludes that addressing the workers living with cancer work-related issues is critical in promoting their wellbeing both in the workplace and health-wise. Further, the focus should be on institutionalising wellness programs through a legal framework and policy guidelines.

THE RELATIONSHIP OF WORKPLACE SUPPORT SYSTEM, WORK-RELATED ISSUES, AND GENERALIZED ANXIETY DISORDER AMONG WORKERS WITH CANCER IN AMMAN, JORDAN

Abstract : Cancer is presently being perceived as a long-term condition due to propels in medicines that increment the survival rate of patients with cancer to as long as ten a long time from the time of the illness. In Jordan, cancer is the moment driving cause of passing after related heart maladies. Uneasiness is among the commonly found psychiatric sickness in patients with cancer and is frequently dismissed. Around 10% of patients with cancer are influenced by uneasiness around the world. Workplace support system is developed to facilitate and support the patients. It also facilitates the adjustment for the diagnosis and disease treatment. The workplace will never be free without associative anxiety. Work-related factors are the perception of employer for the accommodation of employer for cancer treatment or any other disease, for instance, flexible arrangement of working, job replacement, and meeting with the employer when return to work. Subsequently, this study was pointed to define the relationship between workplace support system (WSS), work-related issues (WRI), and generalized anxiety disorder (GAD) and its related factors among workers with cancer within the Jordanian community. The study was conducted at King Hussein Cancer Centre (KHCC) in Amman, Jordan by using a cross-sectional method. To obtain the sample population of 354 Jordanian workers with cancer were used a proportional sampling technique. Data was collected through a self-administered survey and the analyses were carried out using AMOS and SPSS version 25. Where we gained from the participants a response rate of 100%. The relationship between WSS, WRI, and GAD among workers with cancer was the WSS is negative and significantly associated with GAD while the WRI is positive and significantly associated with GAD. There is a high relationship between the WSS, WRI, and GAD among workers with cancer in Jordan. Where the WSS decreases the GAD will increase. As well as the GAD will increase and the WRI will increase.

Ultrasonic fetal Doppler and growth indices and their relation to immediate neonatal outcomes in different severities of maternal anemia

Abstract : Doppler assessment may lead to intervention that reduces the risk of fetal brain damage. Aim of thestudy: to assess the relation between ultrasonic hemodynamic Doppler indices of middle cerebral and umbilical arteries (PI, RI), growth indices to immediate neonatal outcomes (weight, head & abdominal circumference, APGAR scores at 1 and 5 minutes and neonatal unit admission) in women with mild, moderate and severe anemia during pregnancy. Present study is a clinical prospective study carried out in Al-Elwiya Maternity Teaching Hospital during (January-Jun) 2019, all anemic pregnant women presented to Obstetrical wards in hospitals for emergency cesarean section were the study population. The final sample selected was 120 pregnant women. Ultrasound Doppler velocimetry measurements like middle cerebral artery-peak systolic velocity (MCA-PSV), middle cerebral artery-pulsatility index (MCA-PI), middle cerebral artery-resistance index (MCA-PR), umbilical artery-pulsatility index (UA-PI), umbilical artery- resistance index (UA-RI), middle cerebral artery to umbilical artery- resistance index ratio (MCA/UA-RI) and middle cerebral artery to umbilical artery- pulsatility index ratio (MCA/UA-PI) were measured. Neonates with less than7had low vital activities (appearance, pulse, grimace, activity, and respiration). Mean MCA-PI of anemic pregnant women with severe anemia was 1.4ms which was significantly lower than pregnant women with mild and moderate anemia, also mean MCA/UA-PI of anemic pregnant women with severe anemia was significantly lower than mild and moderate anemia with p-value <0.001. as well as the Mean neonatal HC of anemic pregnant women with severe anemia 30 cm which was significantly lower than HC of pregnant women with mild and moderate anemia (p=0.04), also mean neonatal AC of pregnant women with mild, moderate and severe, Current study showed no statistical difference in Apgar scores between the studies group, The validity results of MCA/UA-PI ratio regarding neonatal HC of anemic pregnant women are Sensitivity (22.2%), specificity (41.7%). Severe maternal anemia is found significantly associated with abnormal Doppler study middle cerebral artery to umbilical artery pulsatility index. so Doppler middle cerebral artery to umbilical artery pulsatility index has limited validity in the prediction of neonatal outcomes in anemic pregnant women.

Analysis of Readiness of General Hospital Dr. FL. Tobing Sibolga in the Implementation of the National Health Insurance Policy: A Qualitative Case Report

Abstract : The national social security system and social security administering body require careful readiness by the health insurance providers, such as hospitals. National health insurance program is a pre-effort financing pattern, meaning that health financing is issued before or not when you are sick. Issues of financial sustainability of pro-poor policies must be maintained to reach universal health coverage even though sometimes many countries experience serious financial problems. Dr. FL. Tobing General Hospital Sibolga is one of the hospitals that provide services for the National Health Insurance. This study was to determine the Readiness of Dr. FL. Tobing General Hospital Sibolga in the Implementation of the 2021 National Health Insurance Policy. This study was a qualitative approach. Eleven informants were involved. Data were collected using interviews, observation, and documentation review. The results found that health personnel resources in implementing the health insurance program have been able to fulfill health services effectively. It is hoped that the head of the provincial health office will carry out the process of monitoring and evaluating the implementation of the health insurance policy.

The Functional Indicators Relationship with Achievement Level of 110-Meter Hurdles Players

Abstract : The sports field has become a civilized phenomenon in most countries of the world. sport games have become one of the important criteria to measure how much the countries are developed. Sports physiology has a great and effective role in the sports field. It is concerned with studying the physiology of internal organs and identifying the effects that occur to internal organs and tissues as a result of sports training. This paper tries to identify the functional indicators that lead to the development of the achievement level of 110 hurdlers players. At the same time, consider the functional indicators of player body (heart rate, systolic and diastolic blood pressure, respiratory rate (that related to the effectiveness of 110 barriers. The target sample in this study was from Al-Qasim Club players from Iraq, the category of applicants for the season (2021-2022), and their number was (10) players. The statistical program SPSS was used to process the results. The statistical analysis showed that there is a significant relationship between some of functional indicators that used in this work and the achievement. The functional indicators that have a significant relationship with achievement were heart and respiratory rate. While as, the relationship between systolic blood pressure and the achievement was week. The final functional indicator was diastolic blood pressure showed there is no relationship between this indicator and the achievement. There is no doubt the regular exercise improves the level of functional indicators in player's body; thus, this will be reflected on the level of achievement.
