Impact of Reinforcement Educational ‎Sessions ‎Regarding Integrated Management of Neonatal and Childhood Illness ‎on ‎Nurses ‎Knowledge in Holy ‎Karbala Governorate

Abstract : IMNCI is the process of caring for all ill children under 5 years of ‎age that combines ‎preventive, ‎promotive and curative actions for leading ‎causes of child deaths [1]. Training of nurses in management of IMNCI cases will provide the ‎knowledge and skills needed to provide health services to healthy children ‎for follow up and children suffering from range of diseases and case ‎management, refer ill children who need urgent referral, prescribe and ‎administer appropriate treatments and providing child ‎caregivers with information about treatment and follow up visits. ‎Implementation of IMNCI demonstrated progress in the quality of care ‎provided for sick children [2]. To evaluate the impact of reinforcement educational ‎sessions ‎regarding the integrated management neonatal and ‎childhood illness ‎on nurses ‎knowledge. A quantitative study (quasi-experimental design) carried out for achieving the objectives of present study, by application of pre-test & post-test approach for both study group and control about nurses working in IMNCI units from 20th September 2020 to 25th April 2022. The study is conducted in the primary health care centers of the four health care sectors in Holy Karbala. Non probability sample (purposive sample) from the total population who meet the criteria of the study during the study interval. A total of (60) nurses selected for the study, (30) participant for study group and (30) for control group. The mean age of study group was 33.5 ± 8.1 years and that of control was 32.8 ± 8.8 years, while males were dominant in study group and controls represented 63.3% and 70%, respectively. Vast majority of the participants, 90%, in the study group and 80% of controls were married. In regard educational achievement (66.7%) and (76.7%) of (study group and control) had achieved nursing institute level of education. Comparison of mean knowledge score of study group and controls for all domains revealed no significant difference in mean scores before education program pre-test between both groups, (P>0.05), a highly significant differences in means score at post-test (1) and post-test (2), at (P<0.001). Within each group, it had been found that the change in overall knowledge scores increased significantly in the study group where the overall mean score for all domains increased from 2.36 ± 0.19 before education sessions to 2.90 ± 0.09 at post-test (1) and almost maintained to 2.87 at post-test (2), with a mean difference of 0.51 and a percentage change of 21.8%, the change was highly significant, (P<0.001). Significant improvement appear clearly among the study group member through the result of their pre-test and post-test, which explain the impact of the educational sessions content on the nurses knowledge.

Effectiveness of Educational ‎Program ‎Regarding the Integrated Management of ‎Neonatal and Childhood Illness ‎on Nurses ‎Practices in Holy ‎Karbala

Abstract : Integrated management of neonatal and childhood illnesses (IMNCI) is a number of guidelines build ‎up by the World Health Organization (WHO) in 1992 to improve the assessment, classification, and ‎the individual management of the common causes of death among children ‎under the age of five [1].‎ The development of the IMCI strategy was based on three main components, improving the performance of health care providers, enhancing the performance and management structure of the health system and strengthening family and community practices [2]. To evaluate the impact of educational ‎program ‎regarding the integrated management of ‎neonatal and childhood illness ‎on nurses ‎practices‎. In control group, the change was statistically insignificant where the mean score before education was 2.29 ± 0.17, at posttest 1 was 2.30 ± 0.16 and at posttest 2 it was 2.31 ± 0.16 with a mean difference of only 0.02 and the percentage change was 0.9%, (P>0.05). Significant improvement appear clearly among the interventional group member through the result of their pre-test and post-test, which explain the effectiveness of the reinforcement educational program session content on the nurses practices.

Effectiveness of Health Promotion in Anana Moane and Kalambe Sala in Wakatobi: Community Intervention Based on Local Wisdom in the Prevention of HIV/AIDS Infection

Abstract : The number of people with HIV continues to increase and it is a common concern of the government and society to be able to realize zero new cases by 2030. There are many programs carried out by the government, one of which is increasing education about HIV/AIDS to adolescent groups. To analyze the effectiveness of Health promotion on Anana Moane and Kalambe Sala: Community Intervention Based on Local Wisdom in the Prevention of HIV/AIDS Infection in Wakatobi. Using a quasi-experimental method with a pret test design and a post test two group design to analyze the differences between the two intervention groups. The population is Anana Moane and Kalambe Sala members of the community in Wakatobi aged 17-25 years as many as 68 people. Data analysis using Normalized N-Gain and Independent T tests to see the methods that had the most significant effect on changes in participants' knowledge about HIV/AIDS. There was a significant influence of Health promotion carried out in 2 intervention groups on the knowledge and attitudes of Anana Moane and Kalambe Sala regarding HIV/AIDS Prevention Efforts, but the most influential health promotion was an effective training method in increasing the knowledge and attitudes of Anana Moane and Kalambe Sala. Adolescents' knowledge of HIV/AIDS is still limited, so it requires efforts to approach and educational methods according to local culture as explained to Anana Moane and Kalambe Sala to increase knowledge and change attitudes so that new cases of HIV/AIDS, especially in adolescents, can be reduced.

COMPARTIVE STUDY OF ENDOSCOPIC SINUS SURGERY WITH AND WITHOUT PARTIAL MIDDLE TURBINATE RESECTION

Abstract : The main goals of endoscopic sinus surgery are providing a sufficient drainage and ventilation for paranasal sinuses. Partial resection of the middle turbinate is sometimes necessary to provide good surgical access to the middle meatus. To evaluate the surgical access and patency of middle meatus with clinical outcomes of endoscopic sinus surgery with and without partial middle turbinate resection. A prospective study has been conducted in otolaryngology department at Al-Immamain al- Kadhymain teaching hospital from August 2017 to September 2018 on 24 selected patients with bilateral chronic rhinosinusitis without nasal polyposis, didn’t respond to medical treatment. They managed by endoscopic sinus surgery and divided randomly into two groups: Group I with partial middle turbinate resection including 14 patients (28 sides operated). Group II with middle turbinate preservation including 10 patients (20 sides operated). Follow up was done weekly during 1st month, every other week during 2nd month and once during 3rd month. The patency of middle meatal antrostomy was 100% in group1 and 75% in group2. Adhesion was 3.6% in group1 and 30% in group 2. The duration of operation was shorter in group 1. Over all, symptoms improved in both groups. Partial middle turbinate resection can result in significant long-term patency of middle meatus and decrease in adhesions & shortening surgical time without any significant adverse effect, at same time preserve partially as surgical landmark.

Detection of ESBL Production and Antibiotic Sensitivity Pattern of Enterobacteriaceae Causing Urinary Tract Infection

Abstract : Despite the widespread availability of antibiotics, urinary tract infection (UTI) remains the most common bacterial infection in the human population. Current knowledge of the organisms that cause UTI and their antibiotic susceptibility pattern is mandatory to ensure appropriate therapy. Production of extended-spectrum β-lactamases (ESBLs) is an important mechanism of resistance in Enterobacteriaceae. Knowledge of their prevalence is essential to guide appropriate antibiotic therapy. Aim of the present study was to determine the frequency of ESBLs in uropathogenic Enterobacteriaceae and their antibiogram. A total of 300 uropathogenic Enterobacteriaceae isolated from a teaching hospital. Antimicrobial susceptibility testing was performed by “Kirby Bauer's” disc diffusion method according to “Clinical Laboratory Standard Institute” (CLSI) guidelines. These isolates were tested for ESBL phenotype by CLSI phenotypic confirmatory test (CLSI-PCT). It was found that 53% of the isolates were ESBL producers. E. coli (56%) and Klebsiella species (35%) were the predominant. Nitrofurantoin, piperacillin-tazobactum and amikacin were found to be the most effective antibiotics. ESBL producers were more resistant to cephalosporins, fluoroquinolones, piperacillin-tazobactum, amoxyclav and cotrimoxazole compared to non-ESBL producers. Continued antibiotic surveillance, infection control measures and an effective antibiotic policy are required to address the problem of ESBL associated infections.

Evaluation of distally based bipedicled flap for reconstructing post-burn finger nailfold and finger nail deformities

Abstract : One of common post-burn hand deformities is finger nailfold contracture and deformity. To evaluate the versatility of distally based bipedicled flap technique in correction of post-burn finger nailfold reconstruction. 24 fingers, of nine patients, with postburn finger nailfold deformity, submitted to reconstruction from January 2013 to June 2014, in Al Wasity hospital in Baghdad, using distally based bipedicled flap. A distally based bipedicled flap was designed over the dorsum of the distal phalanx, with fishtail darts placed on each side. The donor area was covered with FTSG (taken from the groin) and fixed with a tie-over bolster dressing. After six months follow-up, most of patients showed satisfactory results regarding both contracted finger nailfold and nail growth. There was no case of total flap necrosis. Apart from minor complications, such as slight bulky flap and hyperpigmented skin graft in flap donor site, no major complications were observed. The distally based bipedicled skin flap is shown to be a simple, reliable, one stage, and effective technique to reconstruct most cases of postburn finger nail fold contracture and deformity. It has the advantage of correcting the associated nail growth deformity through the same surgical procedure with minimum complications.

Occurrence of SARS-COV-2 Variants and Their Relationship to Disease Severity in a Sample of Iraqi Patients

Abstract : There are thousands of variations of the coronavirus virus, which affects the epidemic of this disease; therefore, monitoring these variables is necessary to understand the course of the epidemic and the impact of the type of variations on the severity of the disease. Coronavirus disease is a recent viral disease that has caused a modern global health pandemic since the influenza pandemic in 1918. The study was conducted by collecting nasal swabs of 100 patients with positive samples of COVID-19 virus in cooperation with the Central Public Health Laboratory (CPHL) and Imam Ali Hospital in Baghdad. The SARS-CoV-2 Variant Typing Real-TM (Real-Time PCR kit for the qualitative identification of SARS-CoV-2 Variant mutations in the Spike protein/ Sacace Biotechnologies com./ Italy) diagnostic kit was used to determine the viral genomic information of the samples. The 100 positive Covid-19 PCR template samples' results revealed four different types of recognized variations. Unknown variants 39 made up the greatest portion of the sample (39%; 95%CI: 29.0 - 49.0), followed by 19 OMICRON variant B.1.1529 (19%; 95%CI: 12.0 - 27.0), 17 Brazilian/Gamma variant P.1, 15 South Afrian/Beta variant B.1.351, and 10 British /Alpha variant B.1.1.7. When using the SARS-CoV-2 Variant Typing real -TM diagnostic kit to determine the type of variant, we have 39 unknown variants, which is due to the occurrence of mutations in the genetic structure of this virus. These mutations will have many negative effects on the molecular and serological tests, so the study sample, which consisted of 100 samples, was positive for the PCR test.

The Effect of COVID-19 on Mental Health in the Elderly

Abstract : The COVID-19 pandemic that is currently occurring is causing a mental health crisis not only in young adults but can occur in the elderly, limitations in terms of access and health care facilities, reduced contact with family or relatives, lack of social interaction has the potential to develop into psychological problems, not being impossible could further become a more serious mental issue. Ideally the elderly are more mature than other age groups in dealing with problems related to psychology, but in fact the overwhelming burden of psychological pressure can affect the mental of the elderly. The most difficult situation faced by the elderly since the pandemic occurred is the result of not having sufficient resources to handle the stress and psychological pressure they experience, here the role of medical personnel is to focus and be involved in the importance of a non-pharmacological approach that is more effective than just pharmacotherapy. in the management of mental emotional disorders related to this pandemic situation. Seriousness and assistance from various parties and related elements is needed. The roles of family, health workers, government, and the elderly as objects are obliged to work together to overcome mental and psychological burdens that arise.

ANORECTAL NEISSERIA GONORRHOEAE INFECTION IN HIV PATIENT: A CASE REPORT

Abstract : Gonorrhoea is a common sexually transmitted disease (STD) that affects men and women, caused by Neisseria gonorrhoeae which is transmitted through mucosal secretions. Although considered as an STD, in rare cases, Neisseria gonorrhoeae (NG) can also be affect extragenital locations such as the rectum or anorectal. The increasing number of cases of anorectal gonorrhoea among men is likely due to the patient being bisexual or homosexuals. The presence of gonorrhoea at any site also increases the risk of transmission and the risk of becoming infected with HIV. Laboratory diagnosis of gonococcal infection depends primarily on the identification of NG in the infected site by microscopic examination of staining, culture, or by genetic detection of the organism. Treatment of anorectal gonorrhoea is similar to urogenital gonorrhoea therapy where combination of antibiotic therapy is still the first line treatment. This paper reports a case in a 26-year-old male patient diagnosed with anorectal gonorrhoea in an HIV patient who showed clinical improvement with single dose Azithromycin 2 g monotherapy for 4 weeks.

Prevalence of refractive errors among Saudi population: A systematic review and meta-analysis

Abstract : Refractive errors are common globally with an estimated number of 2.3 billion people suffering from impaired vision secondary to uncorrected refractive errors. This study aimed to estimate a piece of proper evidence about the overall prevalenceof refractory errors within the kingdom. A process of collection of relevant key words was conducted followed by database search. All data were analyzed with R software version 4.0.2. Using a “meta” package. The event rate and the corresponding 95% confidence interval (CI) was used to assess the prevalence of refractive errors in the study. The sample size was 16850 individual with the highest sample size was 5176 and the lowest one was 162. The male prevalence in our sample was 55%. All the included papers were published after 2010 and were of fair quality. Eleven studies of 12,121 individuals reported the overall prevalence of different refrative erros. The pooled overall prevalnce rate was 27.28% (95% CI= 18.29-40.69) (Figure 2A); ranging from 4.55% to 72.20% among individual studies. This wide range of the reported prevalence rates was evident with a significant heterogenity among the included studies (I2= 99%; P-value< 0.001). A huge part of the high rates might be attributable to medical students. Moreover, myopia was the most common refractive error followed by astigmatism and hyperopia.
